申请人前期结果表明,PD-L1在月经周期不同时期的内膜和早期妊娠蜕膜组织中呈现动态表达模式,且注意到PD-L1在蜕膜细胞的细胞质,尤其是细胞核中有较高水平的定位,提示蜕膜细胞细胞核内定位的PD-L1可能作为转录因子和RNA结合蛋白,对蜕膜发育过程中高度动态表达的各类因子起到关键的转录和转录后调控作用。本研究拟利用ESC中PD-L1敲降或敲除细胞、动物模型和复发性流产蜕膜组织的类器官培养模型,运用流式细胞术、转录组测序、染色质免疫共沉淀(ChIP-seq)、翻译组 Ribo-seq、RNA免疫共沉淀测序(RIP-seq)、RNA 免疫共沉淀联合定量PCR(RIP-qPCR)和蛋白质组学分析等技术手段,按照蜕膜分化时程,系统研究PD-L1通过调控蜕膜发育过程中前炎症反应和免疫耐受参与子宫内膜容受性建立的机制。  

Objectives of Study:

Preliminary results from the applicant indicate that PD-L1 exhibits a dynamic expression pattern in endometrial tissues across different phases of the menstrual cycle and in early pregnancy decidual tissues. It was noted that PD-L1 shows high levels of localization in the cytoplasm and particularly in the nucleus of decidual cells, suggesting that nuclear-localized PD-L1 in decidual cells may function as a transcription factor and RNA-binding protein, playing a key role in transcriptional and post-transcriptional regulation of various highly dynamic factors during decidual development. This study plans to utilize PD-L1 knockdown or knockout embryonic stem cell (ESC) models, animal models, and organoid culture models derived from recurrent pregnancy loss (RPL) decidual tissues. Techniques such as flow cytometry, transcriptome sequencing (RNA-seq), chromatin immunoprecipitation sequencing (ChIP-seq), ribosome profiling (Ribo-seq), RNA immunoprecipitation sequencing (RIP-seq), RNA immunoprecipitation combined with quantitative PCR (RIP-qPCR), and proteomics analysis will be employed. Following the timeline of decidual differentiation, this study aims to systematically investigate the mechanism by which PD-L1 participates in establishing endometrial receptivity by regulating pro-inflammatory responses and immune tolerance during decidual development.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

组织样本采集: “在患者知情同意后,于特定月经周期窗口期(分泌中期)或妊娠早期,采集清宫、宫腔诊刮、宫腔搔刮手术或宫腔镜手术中刮取的送病检后剩余的内膜组织以及临床检测后剩余的患者血液标本。取样后立即分为两部分:一部分置于液氮速冻,用于分子生物学分析;另一部分置于特定固定液(如多聚甲醛)中,用于组织学分析。” 临床数据采集: “通过统一的《临床信息调查表》采集患者的年龄、月经史、孕产史、既往病史、手术史、用药史等。所有临床数据由两名研究人员独立录入核对。” 电子化与数据库: “所有临床数据使用加密的电子病例报告表录入,并存储于受密码保护的独立研究数据库(如使用RedCap或本地安全服务器)。实验原始数据(如图像文件、测序下机数据)存储于实验室专属服务器,并定期备份至异地安全存储设备。” 样本标识与追溯: “采用唯一匿名化编码对样本进行标识(如RPL-001-EM),该编码同时链接临床信息数据库和样本库。建立样本追踪日志,记录样本的采集、处理、储存位置和使用情况,确保全程可追溯。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Tissue Sample Collection: 1. Following informed patient consent, endometrial tissue remaining after pathological examination—obtained from procedures such as uterine evacuation, diagnostic curettage, endometrial scratching, or hysteroscopic surgery during specific menstrual cycle windows (e.g., mid-secretory phase) or early pregnancy—as well as residual patient blood samples remaining after clinical testing, are collected. 2. Immediately after collection, samples are divided into two portions: one is snap-frozen in liquid nitrogen for molecular biological analysis, and the other is placed in a specific fixative (e.g., paraformaldehyde) for histological analysis. Clinical Data Collection: 1. Patient information, including age, menstrual history, obstetric and gynecological history, past medical history, surgical history, and medication history, is collected using a standardized "Clinical Information Questionnaire." 2. All clinical data are independently entered and cross-verified by two researchers. Digitization and Database: 1. All clinical data are entered via encrypted electronic case report forms and stored in a password-protected, standalone research database (e.g., using RedCap or a local secure server). 2. Experimental raw data (such as image files and sequencing output) are stored on a dedicated laboratory server and regularly backed up to off-site secure storage devices. Sample Identification and Traceability: 1. A unique anonymized code (e.g., RPL-001-EM) is used to label each sample. This code links both the clinical information database and the specimen repository. 2. A sample tracking log is maintained to record the collection, processing, storage location, and usage of each sample, ensuring full traceability throughout the process.
