本研究旨在通过回顾性分析本中心较大样本量的PNF患者的临床资料，系统性描述其临床特征与诊疗过程，并基于客观解剖路径进行分组比较，以期为该病的早期诊断、精准评估和个体化外科干预策略提供高级别临床证据。

Objectives of Study：

This study aims to systematically delineate the clinical characteristics and management of perianal necrotizing fasciitis (PNF) through a retrospective analysis of a relatively large patient cohort from our center. By comparing groups defined by objective anatomical pathways of spread, we seek to provide high-level clinical evidence to inform early diagnosis, precise assessment, and individualized surgical intervention strategies for this disease.

Inclusion criteria

排除标准：

1.继发性感染（腹腔感染继发会阴扩散/外伤或术后吻合口瘘导致感染）。 2.特殊人群：妊娠期妇女、免疫缺陷（HIV/CD4＜200/μL，器官移植术后）。 3.病历资料缺失或不完整者。

Exclusion criteria：

1. Cases of secondary infection (i.e., perineal spread secondary to intra-abdominal infection, or infection caused by trauma or postoperative anastomotic leakage). 2. Special populations including pregnant women and immunocompromised patients (e.g., HIV infection with CD4 count <200/μL, post-organ transplantation). 3. Patients with missing or incomplete medical records.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	1. 病例记录表 (CRF)：本研究使用自行设计的标准化《肛周坏死性筋膜炎回顾性数据提取表》作为CRF。该表格涵盖了人口学、基础病、临床表现（包括LRINEC评分）、实验室/影像学结果（特别是用于解剖分组的CT/MRI特征）、治疗细节（手术、药物）及预后指标等全部研究变量。 2. 电子采集和管理系统 (EDC)：数据采集流程为：由两名研究者经培训后，独立依据CRF从医院信息系统中提取数据并录入初始电子表。通过双人交叉核对与争议解决机制确保数据准确性。最终数据导入SPSS软件建立锁定数据库。所有数据均经去标识化处理，并存储于安全加密的医院内部设备，实行严格访问控制。
Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：	1. Case Report Form (CRF): A self-designed, standardized Retrospective Data Extraction Form for Perianal Necrotizing Fasciitis was used as the CRF. This form encompassed all study variables, including demographics, underlying diseases, clinical manifestations (including the LRINEC score), laboratory/imaging results (particularly CT/MRI features used for anatomical grouping), treatment details (surgical and pharmacological), and outcome measures. 2. Electronic Data Capture (EDC) System: The data collection process was as follows: Two trained researchers independently extracted data from the hospital information systems according to the CRF and entered them into a preliminary electronic form. A dual-entry cross-verification process with a dispute resolution mechanism was employed to ensure data accuracy. The finalized data were imported into SPSS software to establish a locked database. All data were de-identified and stored on securely encrypted, internally managed hospital devices with strict access controls.
