研究目的：

1.基于信息框架构建CHF患者就医行为指导方案。 2.初步验证CHF患者就医行为指导方案的可行性及应用效果。  

Objectives of Study：

1.Develop an information framework-based healthcare-seeking behavior guidance program for CHF patients. 2.Preliminarily validate the feasibility and application effectiveness of the healthcare-seeking behavior guidance program for CHF patients.

纳入标准：

Inclusion criteria

排除标准：

1.存在语言沟通障碍、无法正常交流；
2.既往有精神病史；
3.合并恶性肿瘤；
4.心脏移植术后；

Exclusion criteria：

1. Presence of language or communication barriers impeding normal interaction; 2. History of psychiatric disorders; 3. Comorbid malignant tumors; 4. Status post cardiac transplantation.
