研究目的：

1.本研究旨在基于旅程地图构建COPD(慢阻肺）患者健康管理，通过系统化梳理患者从疾病诊断、急性加重管理、稳定期维持到康复支持的全过程，明确各阶段的关键节点及医疗需求。 2.基于旅程地图，优化健康管理流程，提升患者自我管理能力及生活质量，同时为医疗机构提供科学化、个性化的慢性病管理工具，推动COPD患者全程化健康管理模式的实践与应用。  

Objectives of Study：

1. This study aims to establish a health management framework for COPD (Chronic Obstructive Pulmonary Disease) patients based on journey mapping. By systematically mapping the entire patient journey—from disease diagnosis, acute exacerbation management, stable-phase maintenance, to rehabilitation support—it identifies key milestones and healthcare needs at each stage. 2. Leveraging the journey map, the study optimizes health management processes to enhance patients' self-management capabilities and quality of life. Simultaneously, it provides healthcare institutions with scientific, personalized chronic disease management tools, advancing the implementation and application of a comprehensive health management model for COPD patients.

纳入标准：

Inclusion criteria

排除标准：

1.合并肺结核、气胸、肺癌等其他严重肺部疾病者；2.患有严重的心、脑、肝、肾疾病的患者；3.合并神经及精神疾病的患者；4.有严重听力、言语功能障碍的患者。

Exclusion criteria：

1. Patients with concurrent severe pulmonary diseases such as tuberculosis, pneumothorax, or lung cancer; 2. Patients with severe cardiac, cerebral, hepatic, or renal diseases; 3. Patients with concurrent neurological or psychiatric disorders; 4. Patients with severe hearing or speech impairments.

本研究属于实验性研究，采用随机数字表法，根据入院顺序用SPSS生成98个数字，然后进行98个数字随机分号，将分号再进行随机分组，分为对照组和试验组，连续选取某三甲医院呼吸科符合纳入排除标准的COPD住院患者作为本次研究对象。

This study is an experimental investigation employing a random number table method. Using SPSS, 98 random numbers were generated based on admission sequence. These numbers were then randomly assigned to create two groups: a control group and an experimental group. Inpatient COPD patients meeting the inclusion and exclusion criteria from the respiratory department of a tertiary hospital were consecutively selected as study subjects.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

建立数据库，确定变量后，将数据录入SPSS数据库，此过程由研究者及另外一人分别录入试验结果，然后交换录入结果，分别进行审核。所有数据应用SPSS25.0进行统计分析。计量资料以（±s）表示，组内比较采用配对t检验，组间比较采用独立t检验。计数资料以[n（%）]表示，比较采用x2检验。以P<0.05为差异有统计学意义。

After establishing the database and defining variables, data were entered into the SPSS database. This process involved two researchers independently entering trial results, followed by cross-checking and reviewing each other's entries. All data were statistically analyzed using SPSS 25.0. Quantitative data are expressed as mean ± standard deviation (+/-s). Intra-group comparisons were performed using paired t-tests, while inter-group comparisons employed independent t-tests. Categorical data are presented as [n (%)], with comparisons conducted using chi-square (χ²) tests. A P value < 0.05 was considered statistically significant.
