研究目的：

本研究旨在以跨理论模型为指导，构建动机性访谈对老年高血压合并衰弱患者自我管理的干预方案。通过开展随机对照试验验证干预的有效性及可行性，为促进该人群的健康行为改变提供证据。  

Objectives of Study：

This study aims to develop a Motivational Interviewing-based intervention protocol for self-management in elderly hypertensive patients with frailty, guided by the Transtheoretical Model. A randomized controlled trial will be conducted to examine the effectiveness and feasibility of the intervention, thereby providing evidence for promoting health behavior change in this population.

纳入标准：

Inclusion criteria

排除标准：

1.高血压急症/亚急症； 2.合并有认知功能障碍或严重精神障碍者； 3.视、听与沟通障碍无法配合完成研究者。

Exclusion criteria：

1.Patients with hypertensive emergency or urgency; 2.Individuals with cognitive impairment or severe mental disorders; 3.Those with visual, hearing, or communication impairments that preclude their ability to cooperate with and complete the study.
