研究目的：

明确艾司氯胺酮作为佐剂在老年食管癌胸腹腔镜术后镇痛中的最佳剂量，评估其对术后疼痛、炎症反应、认知功能的影响，优化 ERAS 多模式镇痛方案，同时验证该方案的安全性。  

Objectives of Study：

To identify the optimal dose of esketamine as an adjuvant in postoperative analgesia for elderly patients undergoing thoracoscopic-laparoscopic esophagectomy, evaluate its effects on postoperative pain, inflammatory response and cognitive function, optimize the ERAS multimodal analgesia regimen, and verify the safety of this regimen.

纳入标准：

Inclusion criteria

排除标准：

1. 对苯二氮卓类药物、阿片类药物、丙泊酚、氟马西尼、纳洛酮、氯胺酮等药物及其药物组分过敏或禁忌者； 2.严重心、肺、肾功能不全，合并多种慢性疾病不能耐受手术麻醉以及精神疾病患者； 3. 颅内压或眼内压增高者； 4.有酗酒史、阿片类药物过敏或有该类药物滥用史者； 5. 未控制的严重高血压患者； 6. 研究者认为不适合入组的其他情况； 7.近期有神经系统疾病或手术患者。

Exclusion criteria：

1. Those who are allergic to or contraindicated to benzodiazepines, opioids, propofol, flumaciclib, naloxone, ketamine and their drug components; 2. Patients with severe heart, lung or kidney insufficiency, combined with multiple chronic diseases who cannot tolerate surgical anesthesia, as well as those with mental disorders; 3. Those with increased intracranial or intraocular pressure; 4. Those with a history of alcohol abuse, opioid allergy or abuse of such drugs; 5. Patients with uncontrolled severe hypertension; 6. Other circumstances that the researcher deems unsuitable for enrollment; 7. People who have had neurological diseases or undergone surgery recently.
