研究目的：

本研究由两个相对独立又相互关联的部分构成，旨在从临床观察到基础机制，系统探讨 Lefort I型截骨术后的愈合问题。 第一部分：单中心、回顾性队列研究 目的： 分析 LeFort I 型截骨患者术后骨愈合及稳定性与上颌骨游离骨块移动方向、距离、术中是否植骨及植骨方式等因素的相关性。 第二部分：单中心、前瞻性、观察性研究 目的：定期拍摄 CBCT 观察行清创术后的患者上颌骨软硬组织的改建情况；利用单细胞测序技术，描绘骨愈合不良区域炎性增生组织的细胞图谱与基因表达特征，阐明其对骨愈合微环境的潜在影响机制。  

Objectives of Study：

This study consists of two relatively independent yet interrelated parts, designed to systematically investigate the healing process following LeFort I osteotomy, transitioning from clinical observation to underlying mechanisms. Part 1: A Single-Center, Retrospective Cohort Study Objective: To analyze the correlation between postoperative bone healing/stability in patients undergoing LeFort I osteotomy and factors such as the direction and distance of maxillary free segment movement, the use of intraoperative bone grafting, and the specific grafting techniques employed. Part 2: A Single-Center, Prospective, Observational Study Objective: To periodically assess the remodeling of both hard and soft tissues in the maxilla post-debridement using CBCT imaging. Furthermore, to utilize single-cell RNA sequencing to characterize the cellular composition and gene expression profiles of inflammatory hyperplastic tissue in areas of compromised bone healing, thereby elucidating their potential mechanistic impact on the bone healing microenvironment.

排除标准：

第一部分：回顾性队列研究方案 排除标准： 1.存在外伤、唇腭裂、肿瘤等造成的严重骨性畸形及骨缺损的患者。 2.正颌手术前诊断为存在慢性上颌窦炎的患者。 3.有全身系统性疾病（糖尿病、自身免疫缺陷综合征等）的患者。 4.存在正颌手术禁忌症的患者。 第二部分：前瞻性观察性研究与基础机制研究方案 排除标准： 拒绝签署知情同意书的患者。

Exclusion criteria：

Part I: Protocol for Retrospective Cohort Study Exclusion Criteria: 1. Patients with severe skeletal deformities or bone defects caused by trauma, cleft lip and palate, tumors, etc. 2. Patients diagnosed with chronic maxillary sinusitis prior to orthognathic surgery. 3. Patients with systemic diseases (e.g., diabetes mellitus, autoimmune deficiency syndrome). 4. Patients with contraindications to orthognathic surgery. Part II: Protocol for Prospective Observational Study and Basic Mechanism Study Exclusion Criteria: Patients who refuse to sign the informed consent form.
