研究目的：

明确肾阻力指数（RRI）与术后血清胱抑素 C（CysC）单独及联合应用时，对心脏手术相关急性肾损伤（CSA-AKI）的预测效能，计算各指标及联合指标的受试者工作特征曲线下面积（AUC）、灵敏度、特异度、阳性预测值与阴性预测值，确定最佳预测阈值。  

Objectives of Study：

To clarify the predictive efficacy of renal resistive index (RRI) and postoperative serum cystatin C (CysC) when used alone and in combination for cardiac surgery-associated acute kidney injury (CSA-AKI), calculate the area under the receiver operating characteristic curve (AUC), sensitivity, specificity, positive predictive value, and negative predictive value for each individual index and the combined index, and determine the optimal predictive threshold.

纳入标准：

Inclusion criteria

排除标准：

术前已存在急性肾损伤（符合 KDIGO 标准）或慢性肾病 4-5 期（eGFR< 30ml/min/1.73m²）；有肾移植病史；严重心功能不全（纽约心脏协会心功能分级 Ⅳ 级）；严重肝功能障碍（Child-Pugh C 级）；凝血功能障碍（国际标准化比值 INR>1.5，血小板计数 < 50×10⁹/L）；经食道超声心动图检查禁忌症；急诊手术;二次手术;非窦性心率;术前 24 小时内使用过肾毒性药物（如氨基糖苷类抗生素、非甾体抗炎药）；临床资料不全者。

Exclusion criteria：

1.Pre-existing acute kidney injury (meeting the KDIGO criteria) or stage 4–5 chronic kidney disease (eGFR < 30 ml/min/1.73m²); History of renal transplantation; severe cardiac insufficiency (New York Heart Association [NYHA] functional class Ⅳ); 2.Severe hepatic dysfunction (Child-Pugh class C); coagulation dysfunction (international normalized ratio [INR] > 1.5, platelet count < 50×10⁹/L); contraindications to transesophageal echocardiography; 3.Emergency surgery; reoperation; non-sinus rhythm; 4.Administration of nephrotoxic drugs within 24 hours before surgery (e.g., aminoglycoside antibiotics, non-steroidal anti-inflammatory drugs); 5.Incomplete clinical data.
