本研究旨在通过随机对照试验，评估盐酸右美托咪定鼻喷剂对老年骨科创伤患者围术期焦虑的疗效及安全性，并探索其作为非侵入性术前用药的临床价值，以期为优化老年患者围术期管理提供高质量证据。  

Objectives of Study：

This study aims to evaluate the efficacy and safety of the nasal spray of dexmedetomidine hydrochloride in reducing perioperative anxiety in elderly patients undergoing orthopedic surgery through a randomized controlled trial. It also intends to explore its clinical value as a non-invasive preoperative medication, with the expectation of providing high-quality evidence for optimizing the perioperative management of elderly patients.

纳入标准：

Inclusion criteria

排除标准：

1.认知或沟通障碍：如痴呆、精神疾病或语言障碍，无法配合评估； 2.药物相关禁忌： 3.对右美托咪定过敏史； 4.近期（72h内）使用镇静/抗焦虑药物（如苯二氮䓬类、阿片类）； 5.严重合并症： 未控制的心血管疾病（如心率<50次/分、收缩压<90mmHg）； 肝肾功能不全（Child-PughC级或eGFR<30mL/min）； 严重呼吸系统疾病（需机械通气或氧依赖）； 6.其他排除因素： 急诊手术（需立即干预，无法完成术前给药流程）； 椎管内麻醉禁忌症（穿刺点感染、颅内高压或严重心肺疾病等） 长期使用α2受体激动剂或拮抗剂（如可乐定、米伐西醇）

Exclusion criteria：

1. Cognitive or communication disorders: such as dementia, mental illness or language impairment, making it impossible to cooperate with the assessment; 2. Drug-related contraindications: 3. History of allergy to dexmedetomidine; 4. Recent (within 72 hours) use of sedative/anxiolytic drugs (such as benzodiazepines, opioids); 5. Severe comorbidities: Uncontrolled cardiovascular diseases (such as heart rate < 50 beats per minute, systolic blood pressure < 90 mmHg); Liver and kidney dysfunction (Child-Pugh C grade or eGFR < 30 mL/min); Severe respiratory diseases (requiring mechanical ventilation or oxygen dependence); 6. Other exclusion factors: Emergency surgery (requiring immediate intervention and unable to complete the preoperative medication process); Intra-spinal anesthesia contraindications (infection at the puncture site, intracranial hypertension or severe heart and lung diseases); Long-term use of α2 receptor agonists or antagonists (such as clonidine, mivacurium);
