探讨超声引导下弓状韧带上腰方肌阻滞与腹横肌平面阻滞在剖宫产手术术后镇痛效果的对照研究，为优化剖宫产术后多模式镇痛策略提供高级别循证医学证据。  

Objectives of Study：

A comparative study exploring the postoperative analgesic effects of ultrasound-guided quadratus lumborum block at the arcuate ligament versus transversus abdominis plane block in cesarean section, aiming to provide high-level evidence-based medical evidence for optimizing multimodal analgesia strategies after cesarean delivery.

纳入标准：

Inclusion criteria

排除标准：

区域阻滞 / 椎管内麻醉禁忌症；对研究用药（罗哌卡因、舒芬太尼）过敏；妊娠期糖尿病控制不佳、肝肾功能异常；合并先兆子痫 / 子痫 / HELLP 综合征；慢性疼痛病史或长期使用镇痛镇静药物；认知障碍或精神疾病；

Exclusion criteria：

Contraindications for regional block/spinal anesthesia; allergy to study medications (ropivacaine, sufentanil); poorly controlled gestational diabetes, liver or kidney dysfunction; complicated by preeclampsia/eclampsia/HELLP syndrome; history of chronic pain or long-term use of analgesics and sedatives; cognitive impairment or psychiatric disorders;

产妇取侧卧位，消毒后将凸阵探头放置在L1 横突尖端和T12 肋上方进行矢状旁扫描。超声在腰方肌深面观察到夹在两条高亮筋膜线之间移动的低回声确认膈肌，以及附着在12肋、LAL上方的QL。将针在探头尾缘向平面内插入，直到针尖刺穿腰方肌肌纤维并进入横膈膜和腰方肌肌肉之间的区域，每侧用0.4%罗哌卡因+4mg地塞米松20 mL。

Intervention：

The parturient was positioned in the lateral decubitus position. After disinfection, a convex array probe was placed over the tip of the L1 transverse process and above the T12 rib for a parasagittal scan. The ultrasound, on the deep surface of the quadratus lumborum muscle, observed a hypoechoic area moving between two hyperechoic fascial lines, confirming the diaphragm, as well as the quadratus lumborum attached above the 12th rib and LAL. The needle was inserted in-plane at the tail end of the probe until the needle tip pierced the quadratus lumborum muscle fibers and entered the space between the diaphragm and the quadratus lumborum muscle. Each side received 20 mL of 0.4% ropivacaine with 4 mg dexamethasone.

TAPB组实施双侧腹横肌平面阻滞，产妇取侧卧位，常规消毒铺巾，使用高频线阵探头，频率为8～13 MHz，将探头置于肋缘、髂嵴两处之间，超声下显示腹壁各层肌肉组织后，采用平面内进针技术，分别在两侧腹内斜肌与腹横肌之间的筋膜阻滞实施穿刺，每侧注射0.4%罗哌卡因+4mg地塞米松25 mL。

Intervention：

The TAPB group underwent bilateral transversus abdominis plane block. The parturient was positioned in the lateral decubitus position, with routine disinfection and draping. A high-frequency linear probe with a frequency of 8–13 MHz was used, placed between the costal margin and iliac crest. After visualizing the abdominal wall muscle layers under ultrasound, an in-plane needle technique was employed. The block was performed by puncturing the fascial plane between the internal oblique and transversus abdominis muscles on both sides, with each side receiving an injection of 25 mL of 0.4% ropivacaine with 4 mg dexamethasone.
