研究目的：

1.研发基于湖南人群特点的生活方式评估工具； 2.以互联网行为改变模型为依据，形成健康生活行为改变方案； 3.构建以健康管理中心为主导的数字化生活方式干预模式并评价其在“真实环境”中的应用效果。

Objectives of Study：

1. To develop a lifestyle assessment tool tailored to the characteristics of the Hunan population; 2. To formulate a healthy behavior modification program based on the Internet Behavior Change Model; 3. To construct a digital lifestyle intervention model led by the health management center and to evaluate its application effectiveness in a "real-world" setting.

干预措施：	体检完成后系统基于人工智能技术自动出具既往由专科医生及健康管理中心全科医生制定的生活方式干预目标及方案。健康管理中心全科医生审核后随访护士按照方案给予干预。通过数字化平台采集受检者饮食、体力活动、吸烟以及饮酒相关信息，并给予实时的方案调整。异常指标，例如血压、体力活动等数值由系统触发报警机制，全科医生及时管理。	干预措施代码：
Intervention：	After the physical examination is completed, the system will automatically generate lifestyle intervention goals and plans—previously developed by specialists and general practitioners at the health management center—using AI technology. Following review by a general practitioner from the health management center, follow-up nurses will carry out interventions according to the plan. The digital platform collects information related to the examinee's diet, physical activity, smoking, and alcohol consumption, and provides real-time adjustments to the intervention plan accordingly. Abnormal indicators, such as blood pressure or physical activity levels, trigger an alert mechanism within the system, enabling timely management by general practitioners.	Intervention code：

Intervention：	After the physical examination is completed, lifestyle intervention goals are developed by a general practitioner at the health management center, who then formulates a corresponding lifestyle intervention plan. Follow-up nurses carry out interventions according to the plan. Follow-up is conducted every three months via phone call or text message to perform a rapid dietary assessment and a physical activity assessment, while also gathering current information on the examinee's smoking and alcohol consumption habits. 抽样与分层：从参加体检的单位中，选取12个符合条件（职工人数300-600人）的单位。根据单位规模、主要慢病患病情况、收入水平、工作性质及地理因素等，采用分层抽样法，将12个单位均衡地划分为四个层（即四组），以确保基线特征在层间尽可能平衡。整群随机分配：在每个层内，将所包含的3个单位，采用计算机生成的随机数字序列，以完全随机的方式分配至三种不同的干预组（如：干预组1、干预组2、对照组）。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Sampling and stratification: select 12 eligible units (300-600 employees) from the units participating in the physical examination. According to the size of the unit, the prevalence of major chronic diseases, income level, nature of work and geographical factors, the 12 units were evenly divided into four layers (i.e. four groups) by stratified sampling method, so as to ensure that the baseline characteristics were as balanced as possible between layers. Cluster random distribution: within each layer, the three units are randomly assigned to three different intervention groups (such as intervention group 1, intervention group 2, and control group) using the random number sequence generated by the computer.
