研究目的:

探讨在非快速眼动睡眠期施加低频双耳节拍声音刺激,以增强慢波睡眠并改善失眠障碍成人睡眠质量的效果。  

Objectives of Study:

To examine the effect of slow frequency binaural sound stimulation during NREM sleep for enhancing slow wave sleep and improving sleep quality in adults with insomnia disorder.

Description for medicine or protocol of treatment in detail:

In the stimulation group, during the PSG overnight recording, EEG from all electrodes will be used for real-time detection of sleep stages. The real time EEG data will be recorded and manually observed, for determining whether playing sound stimulation. Data will be recorded in the Noxturnal software, and timestamp adhere to acoustic simulation will be manually remarked during recording. To entrain slow oscillations EEG, the sound is modified as binaural sound with a 3 Hz beat produced by a 200Hz in the left ear channel and a 203 Hz produced in the right ear channel. Participants will be listening to the binaural beats through a bone- transmitted headphones to ensure that each ear receives only its designated frequency, and this step is crucial for the brain to perceive the beat correctly. In the controlled group, polysomnography recording, and slow waves detection will be similarly performed, but no acoustic stimulation will be delivered.

排除标准:

1. 无法提供知情同意的成年人; 2. 患有除失眠障碍外的临床诊断睡眠障碍,如昼夜节律睡眠障碍、中重度睡眠呼吸暂停,或目前正在接受睡眠呼吸暂停、不宁腿综合征治疗者; 3. 患有临床诊断的严重精神健康问题,如精神分裂症、双相情感障碍,或存在显著自杀风险者。 4. 倒班工作

Exclusion criteria:

1. An adult who is not able to provide consent; 2. With clinically diagnosed sleep disorders other than insomnia disorder, such as circadian rhythm disorder, moderate to severe sleep apnea, or currently receiving treatment for sleep apnea, restless leg syndrome; 3. With clinically diagnosed severe mental health problems such as schizophrenia, bipolar disorders, or with significant risk of suicidality; 4. Shift work

The sound type will be binaural sound with pure tones (Left: 200Hz; Right: 203Hz; falling within the 0-3Hz slow delta frequency band of EEG, to stimulate super slow waves) for acoustic stimulation. Sound intensity will be calibrated to above the individual auditory threshold (typically 25-50 dB) to avoid arousals. Once the NREM stage 2 sleep is visually detected by the experimenter and stable for at least 30 seconds (At least 6 seconds of slow wave sleep passing at least 75uV amplitude during the 30 second epoch), the sound stimulation will be initiated by the experimenter. The sound stimulus will be delivered during the first two cycles of sleep. The binaural sound will be played through bone transmitted earphones.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data will be collected on paper CRFs and transcribed into a structured electronic database (Excel/Access). The primary storage is a secure, access-controlled, and encrypted local NAS server within the Chinese University of Hong Kong firewall. All processes are designed to comply with ICH-GCP principles on data quality and confidentiality.
