研究目的：

（1）通过回顾性研究和质性访谈的方式，分析压力性损伤发生及转归影响因素、护理照护现状存在的问题，为构建压力性损伤全程照护方案提供依据。 （2）以慢性疾病轨迹理论为理论指导，前期研究结合文献研究结果，通过小组会议逐步细化完善形成方案初稿，开展德尔菲专家函询，进一步修订形成方案终稿，最终构建“基于慢性疾病轨迹理论的压力性损伤全程照护方案”。 （4）通过随机对照研究，初步应用并评价该方案在压力性损伤患者中的有效性及可行性。  

Objectives of Study：

(1) Through retrospective studies and qualitative interviews, analyze the factors influencing the occurrence and outcomes of pressure injuries, as well as existing problems in nursing care, to provide a basis for developing a comprehensive care plan for pressure injuries. (2) Guided by the chronic disease trajectory theory, the preliminary research, combined with literature review results, gradually refines and develops the initial plan through group meetings. Delphi expert consultations are conducted to further revise and finalize the plan, ultimately establishing a "Comprehensive Care Plan for Pressure Injuries Based on Chronic Disease Trajectory Theory." (3) Through randomized controlled trials, initially apply and evaluate the effectiveness and feasibility of the plan in patients with pressure injuries.

Inclusion criteria

排除标准：

1.严重精神疾病或意识障碍；2.合并严重皮肤病影响伤口判断；3.合并终末期疾病；4.关键变量缺失>=20%。

Exclusion criteria：

1. Severe mental illness or consciousness disorders; 2.Severe skin diseases affecting wound assessment; 3.Terminal illnesses; 4.Key variable missing>=20%.
