研究目的:

1. 明确经颅直流电刺激对脑损伤后认知障碍的临床疗效:本研究旨在系统评估经颅直流电刺激(tDCS)在儿童脑损伤后认知障碍患者中的治疗效果,重点观察其对注意力、记忆力、执行功能等核心认知领域的改善作用,为临床提供科学依据。 2. 探讨经颅直流电刺激(tDCS)的治疗机制:通过近红外光谱技术(Functional near-infrared spectroscopy,NIRS)检测脑血流,探究tDCS在促进脑组织修复、增强皮层兴奋性与可塑性的机制,为理解其治疗原理提供理论支持。 3. 探究近红外脑功能成像技术与儿童评估量表在认知功能恢复过程中的联系。  

Objectives of Study:

1. Clarify the clinical efficacy of transcranial direct current stimulation (tDCS) for cognitive impairment after brain injury: This study aims to systematically evaluate the therapeutic effect of tDCS in children with cognitive impairment following brain injury, focusing on its improvement in core cognitive domains such as attention, memory, and executive function, so as to provide scientific evidence for clinical practice. 2. Explore the therapeutic mechanism of transcranial direct current stimulation (tDCS): Using functional near-infrared spectroscopy (NIRS) to detect cerebral blood flow, this study intends to investigate the mechanism by which tDCS promotes brain tissue repair and enhances cortical excitability and plasticity, thereby offering theoretical support for understanding its therapeutic principles. 3. Explore the correlation between functional near-infrared spectroscopy (fNIRS) and pediatric assessment scales in the process of cognitive function recovery.

Inclusion criteria

排除标准:

1. 合并其他神经系统疾病:如未控制的癫痫、进行性神经退行性疾病。 2. 严重全身性疾病:先天性心脏病、严重肝肾功能不全、免疫缺陷病等可能影响治疗安全性的疾病。 3. 经颅直流电治疗禁忌症:未控制的气胸、活动性中耳炎、严重肺气肿或肺部感染、癫痫、颅骨缺损等。 4. 无法配合治疗:存在严重行为问题(如攻击性行为)或感官障碍(如失明、耳聋),无法完成治疗流程。 5. 其他特殊情况:近4周变更治疗计划、家属拒绝配合随访或存在数据记录不全风险。

Exclusion criteria:

1. Comorbidity with other neurological diseases: Such as uncontrolled epilepsy, progressive neurodegenerative diseases. 2. Severe systemic diseases: Diseases that may affect treatment safety, including congenital heart disease, severe hepatic or renal insufficiency, and immunodeficiency diseases. 3. Contraindications to transcranial direct current stimulation (tDCS): Uncontrolled pneumothorax, active otitis media, severe emphysema or pulmonary infection, epilepsy, skull defects, etc. 4. Inability to cooperate with treatment: Presence of severe behavioral problems (e.g., aggressive behavior) or sensory impairments (e.g., blindness, deafness) that prevent completion of the treatment process. 5. Other special circumstances: Changes in treatment plans within the past 4 weeks, refusal of family members to cooperate with follow-up, or risk of incomplete data recording.

tDCS 组:给予电流强度为 1.0 mA的电刺激。20分钟/次,每周5天。 tDCS 方案:使用 CE/FDA 认证的经颅直流电刺激仪,Ag/AgCl 环形海绵电极(外径5cm,内径1cm,有效面积≈20 cm2),导电介质使用0.9%生理盐水。阳极(+)放置左背外侧前额叶皮层(左 DLPFC,F3 位置),阴极(–)放置右侧眶上区(Fp2),强度:1.0 mA(0.05 mA/cm2,儿童安全上限),20分钟/次,每周5天。

Intervention:

tDCS Group: Electrical stimulation with a current intensity of 1.0 mA is administered, 20 minutes per session, 5 days per week. tDCS Protocol: A CE/FDA-certified transcranial direct current stimulation (tDCS) device is used, along with Ag/AgCl circular sponge electrodes (outer diameter: 5 cm, inner diameter: 1 cm, effective area ≈ 20 cm2). The conductive medium is 0.9% normal saline. The anode (+) is placed over the left dorsolateral prefrontal cortex (left DLPFC, F3 position), and the cathode (–) is placed over the right supraorbital region (Fp2). Stimulation parameters: 1.0 mA (0.05 mA/cm2, within the safe upper limit for children), 20 minutes per session, 5 days per week.

Sham Stimulation Group: The sham stimulation mode of the device is used. An electrical current of 1.0 mA is delivered for 30 seconds at the beginning and end of the stimulation, with 0 mA for the remaining duration. The treatment is administered 20 minutes per session, 5 days per week.
