研究目的：

本研究通过设计随机对照试验，分析早期主动活动对机械通气困难脱机患者的影响，探讨“基于佩尔梅危重患者活动评分量表构建的早期主动活动方案”在此类患者中应用的效果，进而在临床中推广使用。  

Objectives of Study：

This study conducted a randomized controlled trial to analyze the impact of early active movement on patients with mechanical ventilation-related difficulty in weaning. It also explored the effectiveness of the "early active movement plan based on the Perme Critical Illness Activity Rating Scale" in such patients, with the aim of promoting its application in clinical practice.

纳入标准：

Inclusion criteria

排除标准：

1.呼吸系统：高频振荡通气（HFOV）、俯卧位通气患者； 2.骨科系统：不稳定骨折； 3.神经系统：快速发展的神经肌肉疾病，颈髓损伤，重型颅脑损伤； 4.心脑血管系统：需静脉用降压药控制的高血压急症，急性心脑血管疾病； 5.其他：胸部/胸骨或腹部严重的开放性创伤者，确定的未控制的活动性出血者，大面积烧伤，CRRT治疗过程中，下肢动静脉血栓。

Exclusion criteria：

1. Respiratory system: Patients undergoing high-frequency oscillatory ventilation (HFOV) and prone position ventilation; 2. Orthopedic system: Unstable fractures; 3. Nervous system: Rapidly progressing neuromuscular diseases, cervical spinal cord injury, severe traumatic brain injury; 4. Cardiovascular and cerebrovascular system: Hypertensive emergencies requiring intravenous antihypertensive drugs to control, acute cardiovascular and cerebrovascular diseases; 5. Others: Patients with severe open trauma to the chest/rib cage or abdomen, patients with uncontrolled active bleeding that has been determined, extensive burns, during CRRT treatment, lower extremity venous and arterial thrombosis.
