研究目的：

（1）设计维护与提升社区老年人内在能力及关键维度的有效干预方案； （2）基于RCT评价社区老年人内在能力及关键维度干预策略组合的实践效果； （3）基于彩虹模型提出以维护与提升社区老年人内在能力为目标的整合照护模式的策略，为政策制定者和社区工作者提供参考。  

Objectives of Study：

(1)To develop a structured, multicomponent integrated intervention protocol for maintaining and enhancing intrinsic capacity (and its key domains: cognition, psychology, sensory, vitality, and mobility) among community-dwelling older adults. (2)To evaluate the effectiveness, feasibility, and safety of the intervention protocol on the overall intrinsic capacity and its key domains through a randomized controlled trial (RCT). (3)To propose an integrated care practice model, based on the Rainbow Model, aimed at maintaining and enhancing intrinsic capacity, and to formulate actionable implementation strategies and policy recommendations for policymakers and community practitioners.

纳入标准：

Inclusion criteria

排除标准：

1.由于其他疾病引起的认知功能减退（包括且不限于：脑血管、中枢神经系统感染、帕金森症、代谢性脑病、叶酸和维生素B12缺乏以及甲状腺功能减退）;
2.曾患神经系统疾病（卒中、帕金森症、癫痫等）；美国精神病学会《精神障碍诊断及统计手册》第5版（The Diagnost IC and Statist ICal Manual of Mental Disorders，DSM–5）中包含的精神疾病；
3.患有心脏、肺、肝脏、肾脏、造血系统严重疾病；
4.无日常生活活动能力。

Exclusion criteria：

1. Cognitive impairment attributable to other medical conditions (including but not limited to: cerebrovascular disease, central nervous system infections, Parkinson's disease, metabolic encephalopathy, deficiencies in folic acid and vitamin B12, and hypothyroidism); 2. A history of neurological disorders (e.g., stroke, Parkinson's disease, epilepsy) or any mental disorder as defined by the DSM-5. 3. Severe comorbidities affecting major organ systems (e.g., cardiac, pulmonary, hepatic, renal, or hematopoietic systems). 4. Dependence in basic Activities of Daily Living (ADLs).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

CRF信息采集由老年人自评、工作人员评估共同完成。 数据管理使用Epidata软件双人双录入管理数据，使用SPSS软件进行统计分析。
