研究目的：

旨在评估采用手机APP综合管理血压及心血管危险因素模式对控制血压的影响。  

Objectives of Study：

The aim is to evaluate the effect of comprehensive management of blood pressure and cardiovascular risk factors on blood pressure control by mobile APP.

药物成份或治疗方案详述：

对照组采用传统血压控制模式：门诊就诊随访，APP采集血压，无互动交流。干预组要求家中自测血压至少每日2次（早、晚），监控并每周主动干预2次（每周1、4的11:00-13:00发送调整用药信息并要求10小时内确认，发送患教材料）；每次服药时间点提醒；互动设计：鼓励患者进行反馈。两组随访时间均为6个月。 用药方法：1）血压低于160/or100mmHg，首选缬沙坦胶囊（代文）80mg 1/日；2）血压高于160/or100mmHg，首选缬沙坦氨氯地平（倍博特）80/5mg 1/日；3）2周控制不佳，1）中换为倍博特，2）中加用缬沙坦氢氯噻嗪80/12.5mg(半片或1片，根据具体血压判断)；4）2周后仍然控制不佳，加用α/β阻滞剂；5）加用螺内酯。

纳入标准：

Inclusion criteria

排除标准：

1. <60岁；
2. 两次诊室测量血压中任何一次收缩压<140mmHg；
3. 目前正在参与其他血压控制研究或智能手机APP相关研究；
4. 无法使用iOS或Android系统智能手机；
5. 目前应用超过2种降压药；
6. 目前正在进行化疗或放疗；
7. 无法理解中文；
8. 双侧肾动脉狭窄；
9. 无法保证2年后复诊；
10.臂围对于研究所用袖套过大或过小；
11.目前正在接受透析治疗；
12.无法采集完整基线资料；
13.存在可能导致生存时间小于3年的疾病，或过去两年内诊断并治疗的肿瘤并可导致患者无法良好地配合研究过程；
14.无法取得知情同意；
15.存在明显的其他异常体征、实验室检查和临床疾病，根据研究者的判断，不适合参加研究者。

Exclusion criteria：

1. <60 years old;
2. Two systolic blood pressures measured <140mmHg;
3. Currently participating in other blood pressure control studies or smartphone APP related research;
4. Unable to use iOS or Android system smartphones;
5. Currently more than 2 antihypertensive drugs are used;
6. Chemotherapy or radiation therapy is currently underway;
7. Can't understand Chinese;
8. bilateral renal artery stenosis;
9. There is no guarantee of returning after 2 years;
10. The arm circumference is too large or too small for the research sleeve;
11. Currently receiving dialysis treatment;
12. It is not possible to collect complete baseline data;
13. The presence of a disease that may result in a survival time of less than 3 years, or a tumor diagnosed and treated within the past two years, may result in the patient not being able to cope well with the research process;
14. Inability to obtain informed consent;
15. There are obvious other abnormal signs, laboratory tests and clinical diseases, which are not suitable for researchers according to the judgment of the researchers.
