研究目的:

以六分钟步行距离(6MWD)<350米作为运动耐量差的临床对照标准,采用二维超声及剪切波弹性成像技术对IPF患者的股四头肌厚度等超声指标进行测量,以评估IPF患者的运动耐量及明确其潜在的临床应用价值  

Objectives of Study:

A 6-minute walk distance (6MWD) of less than 350 meters was used as the clinical reference standard for poor exercise tolerance. Quadriceps muscle thickness and other ultrasound parameters were measured using two-dimensional ultrasonography and shear-wave elastography in IPF patients to assess their exercise tolerance and determine the potential clinical utility of these measures.

纳入标准:

Inclusion criteria

排除标准:

1.最近1个月内病情恶化、肺功能下降; 2.存在严重的心血管疾病或活动性肺部疾病; 3.各种影响肌肉的伴随疾病、神经系统疾病或单肢优势腿; 4.无法完成六分钟步行测试者。

Exclusion criteria:

1. Disease exacerbation or significant decline in pulmonary function within the past month. 2. Presence of severe cardiovascular or active pulmonary diseases. 3. Any comorbid conditions affecting muscles, neurological disorders, or unilateral limb dominance. 4. Inability to complete the six-minute walk test.

De-identified individual participant data (including baseline characteristics, primary and secondary outcome measures, safety data, and other information necessary for confirmatory analyses, along with the study protocol, statistical analysis plan, and informed consent template) will be made available after the main findings of this study have been published in a peer-reviewed journal. The data will be shared upon reasonable request starting six months after the article’s publication date. Requesters should submit a formal application to the corresponding author of this study (email: 1040843414@qq.com), detailing the research objectives, analysis plan, and team composition. After approval by the study steering committee, the requester will be required to sign a data use agreement committing to use the data only for the agreed-upon, ethics-compliant secondary analysis, with no attempt to identify participants, no commercial use, and no transfer to third parties.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

①股四头肌超声由同一名超声医师独立进行超声测量,且对临床检查结果不知情。为减少弹性成像检查时人为误差,需合理放置探头,减少因外力肌肉形变而引起的测量误差。6MWT由同一临床医生进行测试。 ②数据收集:使用自行设计的调查问卷采集基线及相关检查数据;②数据的录入与核查:建立epidata数据库,有两名经过培训的数据录入员独立进行数据录入,之后进行双录入检查。数据库不一致时,逐项核对原始记录表进行核对;③数据的质量管理:定期抽查部分病例报告表,了解数据录入的质量,分析并处理录入数据存在的问题。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

① Ultrasonography and Functional Testing All quadriceps ultrasonographic examinations were performed by a single sonographer who was blinded to the clinical findings. To minimize artifacts during elastography, the probe was carefully placed to avoid excessive pressure and subsequent muscle deformation. The six-minute walk test (6MWT) was administered by a separate clinician. ② Data Management Data Collection: Baseline and examination data were collected using a customized case report form. Data Entry and Verification: An EpiData database was established. Two trained data entry clerks independently entered all data, followed by a double-entry verification process. Any discrepancies identified were resolved by reviewing the original source documents. Quality Control: A random sample of case report forms was periodically audited to monitor data entry quality, identify issues, and implement corrective actions.
