研究目的：

探讨内镜下逆行阑尾炎治疗术治疗急性非复杂性粪石性阑尾炎的有效性及安全性，为其临床应用提供高等级循证医学证据。

Objectives of Study：

To investigate the efficacy and safety of endoscopic retrograde appendicitis therapy for the treatment of uncomplicated acute appendicitis with appendicolith and provide high-level evidence-based medical support for its clinical application.

纳入标准：

Inclusion criteria

排除标准：

1. 影像学表现提示阑尾黏液囊肿或阑尾肿瘤可能，或疑似其他急腹症； 2. 免疫功能不全（如绝对中性粒细胞计数 < 500/mm³、需要长期接受免疫抑制治疗、正在进行化疗，或艾滋病）； 3. 孕期或哺乳期妇女；4.对研究拟用药物（抗生素、泻剂）过敏或有禁忌症； 5. 入组前已接受过两剂或以上静脉抗生素治疗； 6. 肠结核，或需要积极治疗的炎症性肠病，或严重肠粘连； 7. 严重器官功能障碍或凝血功能异常，无法安全耐受肠镜或有禁忌； 8. 需要积极治疗的恶性肿瘤； 9. 任何需要立即住院治疗或紧急干预的疾病； 10. 合并其他需要抗生素积极治疗的感染性疾病; 11. 预期无法配合完成研究或随访； 12. 复发性阑尾炎; 13. 正参与可能影响本试验结果的其他研究。

Exclusion criteria：

1. Imaging findings suggestive of appendiceal mucocele or suspected appendiceal neoplasm, or other possible acute abdominal conditions; 2. Immunodeficiency (e.g., absolute neutrophil count < 500/mm³, chronic use of immunosuppressive drugs, active chemotherapy, or AIDS); 3. Women who are pregnant or lactating; 4. Allergy or contraindication to proposed antibiotics or laxatives; 5. Initiation of non-operative management with two or more doses of intravenous antibiotics at an outside institution; 6. Intestinal tuberculosis, or inflammatory bowel disease requiring active medical treatment, or severe intestinal adhesions; 7. Severe organ dysfunction or coagulopathy precludes safe tolerance of colonoscopy or is contraindicated; 8. Malignancy requiring active treatment; 9. Any concurrent illness mandating hospitalization or emergent intervention; 10. Another infection that requires treatment with systemic antibiotics; 11. Patients who are expected to be unable to comply with the study protocol or complete the one-year follow-up; 12. Previous episode of appendicitis; 13. Participating in other clinical trials.
