研究目的:

阐明卒中执行功能障碍患者在完成计时起立-行走任务时,大脑认知控制网络与运动控制网络的激活特征和功能连接模式; 评估调控左侧DLPFC兴奋性对改善卒中患者运动想象能力和BCI训练的作用及其神经机制,从脑网络角度揭示提高卒中患者认知-运动功能的可行途径。  

Objectives of Study:

To elucidate the activation characteristics and functional connectivity patterns of the cognitive and motor control networks in stroke patients with executive dysfunction when performing a timed stand-up-walk task;To assess the effect of modulating left DLPFC excitability on improving motor imagery and BCI training in stroke patients and its neural mechanisms, and to reveal feasible ways to improve cognitive-motor function in stroke patients from the perspective of brain networks.

Inclusion criteria

排除标准:

1. 脑卒中急性期病情不稳定或存在进行性卒中、反复脑卒中发作者; 2. 既往或目前有其他严重神经系统疾病(如脑出血、脑外伤、帕金森病)、重度内科疾患(严重心肺肝肾功能不全等)或重度精神疾病者; 3. 合并感觉性失语、严重认知障碍(如痴呆)或视觉/听觉障碍导致无法配合评估和训练者; 4. 既往有癫痫发作史或癫痫家族史(因TMS可能增加癫痫风险,需排除); 5. 近期MRI显示有新的脑梗死灶或大面积梗死形成者; 6. 合并恶性肿瘤、生命预后有限者。

Exclusion criteria:

1. Patients with unstable acute stroke or those experiencing progressive or recurrent strokes; 2. Patients with a history of or current serious neurological diseases (such as cerebral hemorrhage, traumatic brain injury, Parkinson’s disease), severe medical conditions (severe cardiopulmonary, hepatic, or renal insufficiency, etc.), or severe mental illness; 3. Patients with sensory aphasia, severe cognitive impairment (such as dementia), or visual/auditory impairment that prevents them from cooperating with assessment and training; 4. Patients with a history of epileptic seizures or a family history of epilepsy (to be excluded due to the potential increased risk of epilepsy from TMS); 5. Patients whose recent MRI shows new cerebral infarction foci or large-area infarction formation; 6. Patients with malignant tumors and limited life prognosis.

每次刺激采用三脉冲50 Hz成串,重复周期5 Hz,总共提供600发刺激脉冲,刺激强度为个体主动运动阈值的80%,1周5次,持续3周。在每次iTBS刺激后,患者立即进行运动想象BCI训练,训练任务为想象骑自行车10分钟。

Each stimulation used a three-pulse 50 Hz series with a repetition period of 5 Hz, providing a total of 600 stimulation pulses. The stimulation intensity was 80% of the individual's active motor threshold, administered 5 times a week for 3 weeks. Immediately after each iTBS stimulation, the patient underwent BCI motor imagery training, with the training task being to imagine riding a bicycle for 10 minutes.
