研究目的：

本研究旨在以前瞻性、多中心、配对队列设计，系统评估醛固酮水平升高能否作为高血压人群发生主动脉夹层的独立危险因素。研究结果有望为主动脉夹层的风险分层和早期预防提供新的生物标志物和干预靶点。

Objectives of Study：

This study aims to systematically evaluate, through a prospective, multicenter, matched-pair cohort design, whether elevated aldosterone levels can serve as an independent risk factor for aortic dissection in the hypertensive population. The findings are expected to provide a new biomarker and intervention target for the risk stratification and early prevention of aortic dissection.

Inclusion criteria

排除标准：

病例组排除标准： 1）创伤性主动脉夹层； 2）医源性主动脉夹层； 3）需急诊手术的主动脉夹层； 4）已知患有遗传性结缔组织病，如马凡综合征、Loeys-Dietz综合征、血管性Ehlers-Danlos综合征等； 5）已知的嗜铬细胞瘤、库欣综合征等其他继发性高血压原因； 6）严重肾功能不全（eGFR < 30 ml/min/1.73m²），因其可能影响醛固酮和肾素水平； 7）严重肝功能损害（Child-Pugh C级）； 8）妊娠期或哺乳期妇女； 9）无法配合完成研究或提供完整病史资料者。对照组排除标准： 1）任何主动脉疾病史，包括主动脉夹层、主动脉瘤或主动脉壁间血肿； 2）严重心血管疾病史，如近期（6个月内）心肌梗死、不稳定性心绞痛、严重心力衰竭（NYHA IV级）； 3）已知的继发性高血压原因，如肾血管性高血压、嗜铬细胞瘤、库欣综合征等； 4）严重肾功能不全（eGFR < 30 ml/min/1.73m²）或严重肝功能损害； 5）妊娠期或哺乳期妇女； 6）无法配合完成研究或提供完整病史资料者。

Exclusion criteria：

Exclusion criteria for the case group: 1) Traumatic aortic dissection; 2) Iatrogenic aortic dissection; 3) Aortic dissection requiring emergency surgery; 4) Known diagnosis of inherited connective tissue diseases, such as Marfan syndrome, Loeys-Dietz syndrome, vascular Ehlers-Danlos syndrome, etc.; 5) Known secondary causes of hypertension, such as pheochromocytoma, Cushing's syndrome, etc.; 6) Severe renal insufficiency (eGFR < 30 ml/min/1.73m²), as it may affect aldosterone and renin levels; 7) Severe hepatic impairment (Child-Pugh class C); 8) Pregnant or lactating women; 9) Inability to cooperate with the study or provide complete medical history data. Exclusion criteria for the control group: 1) Any history of aortic diseases, including aortic dissection, aortic aneurysm, or aortic intramural hematoma; 2) History of severe cardiovascular diseases, such as recent (within 6 months) myocardial infarction, unstable angina, or severe heart failure (NYHA class IV); 3) Known secondary causes of hypertension, such as renovascular hypertension, pheochromocytoma, Cushing's syndrome, etc.; 4) Severe renal insufficiency (eGFR < 30 ml/min/1.73m²) or severe hepatic impairment; 5) Pregnant or lactating women; 6) Inability to cooperate with the study or provide complete medical history data.
