研究目的：

主要目的 评估新型“膈肌-呼吸肌异步指数”（DRAI，计算为30分钟自主呼吸试验（SBT）结束时测得的TFic与DTF的比值，即DRAI = TFic/DTF）在预测SBT失败方面的诊断准确性。 次要目的 （1）评估SBT期间DTF相对变化（ΔDTF%）和TFic相对变化（ΔTFic%）对撤机失败的预测价值。 （2）确定DRAI、ΔDTF%和ΔTFic%的最佳诊断截断值。 （3）比较DRAI、ΔDTF%和ΔTFic%与传统浅快呼吸指数（RSBI）的预测性能。 （4）通过多变量逻辑回归分析，探究DRAI是否为SBT失败的独立预测因子。  

Objectives of Study：

Primary Objective To evaluate the diagnostic accuracy of a novel "Diaphragm-Respiratory muscle Asynchrony Index" (DRAI), calculated as the ratio of TFic to DTF (DRAI = TFic/DTF) measured at the end of a 30-minute SBT, for predicting SBT failure. Secondary Objectives (1)To assess the predictive value of the relative change in DTF (ΔDTF%) and TFic (ΔTFic%) during SBT for weaning failure. (2)To determine the optimal diagnostic cut-off values for DRAI, ΔDTF%, and ΔTFic%. (3)To compare the predictive performance of DRAI, ΔDTF%, and ΔTFic% with the traditional RSBI. (4)To investigate whether DRAI is an independent predictor of SBT failure using multivariate logistic regression analysis.

纳入标准：

Inclusion criteria

排除标准：

(1) 已知或临床怀疑存在影响膈肌功能的神经肌肉疾病； (2) 已知存在膈神经损伤或膈肌麻痹； (3) 无法获取清晰且可重复的膈肌或肋间肌超声图像； (4) 妊娠； (5) 研究者认为患者存在其他不适合参与研究的情况。

Exclusion criteria：

(1) There are known or clinically suspected neuromuscular diseases that affect diaphragm function; (2) It is known that there is phrenic nerve injury or phrenic paralysis; (3) It is impossible to obtain clear and repeatable ultrasound images of the diaphragm or intercostal muscles; (4) Pregnancy; (5) The researcher believes that the patient has other conditions that make them unsuitable for participating in the study.
