研究目的:

非小细胞肺癌(Non-small Cell Lung Cancer,NSCLC)是肺癌中最常见的病理类型,外科手术作为其重要治疗手段,在延长患者生存时间的同时,也带来了诸多术后康复问题,如身体功能下降、运动耐力减退及生活质量降低等。研究表明,术后适度的体力活动有助于改善患者功能状态、减少并发症,并降低再入院风险。然而,临床观察发现,NSCLC术后患者的体力活动水平普遍较低,存在运动参与不足、依从性差、难以坚持等现实困境,严重影响康复质量和远期健康结局。多理论模型(multi-theoretical model, MTM)是一种整合多种行为改变理论的新兴框架,强调从行为启动和维持两个阶段实施多维度干预,涵盖个体认知、环境调整与社会支持等多方面因素。该模型已在慢性病管理领域展现出良好的适用性和效果,但其在NSCLC术后患者体力活动干预中的应用尚待深入探索。基于此,本研究拟基于MTM构建一套系统、结构化的体力活动干预方案,并通过实证研究验证其在我国NSCLC术后患者中的应用效果,以期为临床护理实践提供理论依据和实用参考,助力患者运动康复和整体健康管理水平的提升。  

Objectives of Study:

Non-small Cell Lung Cancer (NSCLC) is the most common pathological type of lung cancer. Surgery, as a primary treatment modality, can prolong patient survival but also leads to various postoperative rehabilitation challenges, such as declines in physical function, reduced exercise tolerance, and diminished quality of life. Studies have shown that appropriate physical activity after surgery helps improve functional status, reduce complications, and lower the risk of rehospitalization. However, clinical observations indicate that postoperative NSCLC patients generally exhibit low levels of physical activity, facing practical difficulties including insufficient participation, poor adherence, and difficulty in maintaining exercise, which significantly impact rehabilitation quality and long-term health outcomes. The multi-theoretical model (MTM) is an emerging framework that integrates multiple behavior change theories. It emphasizes multi-dimensional interventions across both the initiation and maintenance phases of behavior change, covering factors such as individual cognition, environmental adjustments, and social support. This model has demonstrated good applicability and effectiveness in chronic disease management; however, its application in physical activity interventions for postoperative NSCLC patients remains underexplored. Therefore, this study aims to develop a systematic and structured physical activity intervention protocol based on the MTM and empirically validate its effectiveness among postoperative NSCLC patients in China. The findings are expected to provide a theoretical foundation and practical reference for clinical nursing practice, supporting patient exercise rehabilitation and enhancing overall health management.

Inclusion criteria

排除标准:

1.术前需行新辅助放化疗及免疫综合治疗的患者; 2.存在沟通、意识障碍的患者; 3.骨质疏松、关节炎、周围神经病变等合并骨骼肌肉疾病的患者; 4.患有严重贫血的患者; 5.合并有其他严重威胁生命的疾病; 6.有严重的躯体疾病,导致无法配合的患者。

Exclusion criteria:

1. Patients who need to undergo neoadjuvant radiotherapy and chemotherapy as well as comprehensive immunotherapy before the operation; 2. Patients with communication or consciousness disorders; 3. Patients with bone and muscle diseases such as osteoporosis, arthritis, and peripheral neuropathy; 4. Patients with severe anemia; 5. Patients with other serious life-threatening diseases; 6. Patients with severe physical illnesses that prevent them from cooperating.
