研究目的:

国际上已经开始采用改良的(新一代)在线计算器进行精准白内障联合散光矫正手术的术前设计,然而现有的几种改良公式仅基于外国人群数据设立,并未考虑到中国人群与外国人群角膜屈光状态及眼球生物结构的差异性,因此本研究旨在设计基于中国人群眼球生物测量参数的大基本数据的改良计算器。并明确不同生物测量仪器对于角膜散光测量的差异,在新型计算器中明确不同仪器的数据,进行进一步精准计算。从而在临床上能够更方便的更准确地开展散光矫正手术,进一步提高我国屈光性白内障手术的视觉质量。  

Objectives of Study:

The improved (new generation) online calculator has been used to perform preoperative design of cataract with astigmatism. However, all of the existing improved formulas are based on foreign population data, and dont consider the difference of the refraction and the ocular biological structures between the Chinese population and the aliens. The purpose of this study is to design an improved calculator based on the large basic data of the ocular biometric parameters in Chinese population. The differences of corneal astigmatism measurements between different biometric instruments were compared, and the data of different instruments were clarified in the new calculator. Therefore, astigmatism correction can be carried out more conveniently and accurately, and the visual quality of refractive cataract surgery in China can be further improved.

药物成份或治疗方案详述:

1、中国人群眼生物测量值及角膜前、后表面散光的大数据测量及计算 1)选用A超、IOL master、Lenstar 测量中国人群眼轴长度、前房深度、WTW等,了解各常用测量仪器之间的一致性 2)选择自动角膜曲率计、IOL master、Lenstar以及Pentacam对角膜前表面曲率进行测量,了解各常用测量仪器之间的一致性。 3)主要采用Pentacam对角膜后表面曲率进行测量,并获得总角膜曲率值。通过不同公式计算角膜曲率,比较各种测量值间的差异(包括K-master、simk、true net power、total corneal refractive power、barrett-IOL master、barrett-simk等)。选取一定的参数,例如年龄,眼别,角膜厚度,前房深度,眼轴长度以及角膜前表面曲率及轴位,与角膜后表面/总角膜屈光值进行多元回归分析,以获得相关回归公式并简化为计算量表,使临床医师可以在没有后表面测量仪器的情况下简单计算角膜后表面曲率。而基于本项研究,可以对于厂商所提供的在线计算器分析进行简单的修正。 4)根据Pentacam及IOLmaster 对以角膜顶点及瞳孔中心不同位置为中心的角膜曲率进行分析,了解Kappa角或者Alpha角对于散光测定的影响。 2、角膜散光值与人工晶体散光值换算比值 基于常规白内障手术患者研究,采用高斯公式计算基于不同前房深度参数的Toricity,同时也采用不同人工晶体计算公式所获的人工晶体有效位置(Effective Lens Position,ELP)进行Toricity计算。采用统计分析不同计算方法的结果,了解不同计算方法的差异性是否有统计学差别。在此研究基础上,获得相对准确的动态角膜散光值与人工晶体散光值换算比值。 3、Toric在线计算器的设计 根据上述研究结果,初步设计出基于中国人群的Toric计算器(app),并通过集团进行多中心研究,对计算器用于临床Toric植入术患者进行准确性研究。最终将在线计算器用于临床使用。 

Description for medicine or protocol of treatment in detail:

1. Measurement and calculation the big data of ocular biometric value astigmatism of anterior and posterior corneal surface in Chinese population 1) Measuring axial length, anterior chamber depth and WTW of Chinese population with A-mode ultrasound, IOL master and Lenstar to understand the consistency of the common measuring instruments. 2) The curvature of the front surface of the cornea was measured by automatic corneal curvature meter, IOL master, Lenstar and Pentacam to understand the consistency between the commonly used measuring instruments. 3) Pentacam was used to measure the posterior surface curvature of the cornea and the total corneal curvature. The curvature of the cornea was calculated by different formulas (including K-master, SimK, true net power, total corneal refractive power, barrett-IOL master, barrett-simk and so on). Select certain parameters, such as age, eye, corneal thickness, anterior chamber depth, axial length, and anterior corneal curvature and axis, and make a multiple regression analysis with the posterior corneal and total corneal refractive values to obtain relevant regression formulas and simplify the calculation scale, so that the clinician can have the back surface keratometric power. Based on this research, the online calculator analysis provided by the manufacturer can be revised simply. 4) According to Pentacam and IOLmaster, the corneal curvature at the center of the corneal apex and the center of the pupil was analyzed to understand the effect of Kappa angle or Alpha angle on astigmatism. 2. Conversion ratio of cornea astigmatism and intraocular lens astigmatism. Based on the patients with conventional cataract surgery, the Gauss formula is used to calculate Toricity of different anterior chamber depth. At the same time, the effective position of the IOL(Effective Lens Position, ELP) obtained by different IOL formulas is also used for Toricity calculation. We will understand if there is statistical difference in different calculation methods. Based on this study, we will get a relatively accurate conversion ratio between dynamic corneal astigmatism and astigmatism. 3Design Toric online calculator Based on the results of the study, the Toric calculator (APP) based on the Chinese population will be initially designed, and a multicenter study was carried out by the group to study the accuracy of the calculator for the patients with clinical Toric implantation. In the end, the online calculator will be used for clinician. 

纳入标准:

Inclusion criteria

排除标准:

1、中国人群眼生物测量值及角膜前、后表面散光的测量及计算
排除标准:1)有角膜病变,眼部炎症, 2)中重度干眼患者,3)有眼部外伤史、眼部手术史,4)不能配合或固视不佳
2、角膜散光值与人工晶体散光值换算比值
排除标准:1)全身情况差,不能耐受手术者,2)有角膜病变,眼部炎症、翼状胬肉、葡萄膜疾病、视网膜疾病、青光眼等眼部病变者,3)有眼部外伤史、眼部手术史

Exclusion criteria:

1. Measurement and calculation of ocular biometric value astigmatism of anterior and posterior corneal surface in Chinese population:
1) with corneal lesions, ocular inflammation;
2) moderate or severe dry eyes;
3) with the history of ocular trauma or eye surgery;
4) poor coordination or poor fixation.
2. Conversion ratio of cornea astigmatism and intraocular lens astigmatism:
1) poor general condition, intolerance of operation;
2) with corneal lesions, ocular inflammation, pterygium, vineal disease, retinopathy, glaucoma, and other eye lesions;
3) with the history of ocular trauma or eye surgery.
