探讨节奏型听觉刺激联合分带行走训练对脑卒中偏瘫患者步行能力(步速、步频、步态对称性等)的改善效果。  

Objectives of Study:

Exploring the improvement effect of rhythmic auditory stimulation combined with zonal walking training on walking ability (pace, step frequency, gait symmetry, etc.) in stroke patients with hemiplegia.

纳入标准:

Inclusion criteria

排除标准:

1. 患者脑卒中同时并发严重的心血管系统相关疾病,如,严重心衰、心绞痛发作、急性心肌梗死、房颤、严重肺气肿等; 2. 患者存在严重感染,或有严重的肺、肝、肾、内分泌系统和造血等重要系统功能问题及严重的骨质疏松、骨折或关节病等合并症患者; 3. 血流动力学不稳定患者,即未受控制的高血压等; 4. 存在严重言语、视力、听力障碍或精神障碍等影响认知功能;

Exclusion criteria:

1. Concurrent severe cardiovascular diseases at the time of stroke, such as severe heart failure, active angina pectoris, acute myocardial infarction, atrial fibrillation, or severe emphysema. 2. Presence of severe infection, significant dysfunction of major systems (e.g., pulmonary, hepatic, renal, endocrine, or hematopoietic), or comorbidities such as severe osteoporosis, fractures, or arthropathy. 3. Hemodynamic instability, including uncontrolled hypertension. 4. Severe aphasia, visual or hearing impairment, or psychiatric disorders that significantly impact cognitive function.

Split-belt treadmill training was administered at a belt speed ratio of 2:1 (fast/slow). The fast belt speed was set according to the individual's overground fast walking speed. The sessions, which incorporated a metronome providing rhythmic auditory stimulation, lasted 25 minutes each, and were conducted three times per week for three weeks.

Randomization was conducted using a web-based software tool (http://www.randomizer.org). Eligible patients were allocated in a 1:1:1 ratio to one of the three groups. The randomization sequence was generated and concealed by a statistician not involved in patient recruitment. The allocation information was kept in sealed, opaque envelopes until the intervention commenced.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection and management will be performed using a two-component system: (1) paper-based Case Report Forms (CRFs) for initial data recording at the study sites, and (2) a web-based Electronic Data Capture (EDC) system for data entry, storage, and management. For this study, we will utilize the ResMan EDC system (www.medresman.org) to ensure data integrity and security.
