研究目的：

基于全科医学慢病管理视角下老年共病患者衰弱的运动干预研究，可预防或延缓老年慢性共病患者衰弱的发生发展，增加老年患者全身肌肉量，改善其平衡能力、力量与肌肉功能，降低跌倒、残疾、住院和死亡等不良健康结局的风险，提高生活质量，达到成功老龄化；制定具有运动锻炼视频，为临床实践提供参考依据。  

Objectives of Study：

The study on exercise intervention for frailty in elderly patients with comorbidities from the perspective of general practice chronic disease management can prevent or delay the occurrence and progression of frailty in elderly patients with chronic comorbidities, increase their total body muscle mass, improve their balance ability, strength and muscle function, reduce the risk of adverse health outcomes such as falls, disability, hospitalization and death, enhance their quality of life, and achieve successful aging. Additionally, it will develop exercise videos to provide a reference for clinical practice.

纳入标准：

Inclusion criteria

排除标准：

第一部分： 1. 痴呆、思维不清、各种精神病患者； 2. 语言表达不清或语言沟通障碍、听力问题、认知障碍。 第二部分： 1. 痴呆、思维不清、各种精神病患者； 2. 失能卧床者或临终患者； 3. 慢性病急性发作期或突发其他急症的患者； 4. 语言表达不清或语言沟通障碍、听力问题、认知障碍。

Exclusion criteria：

Part 1: 1. Dementia, confused thinking, various psychiatric disorders; 2. Impaired language expression or communication difficulties, hearing problems, cognitive impairment. Part 2: 1. Dementia, confused thinking, various psychiatric disorders; 2. Disabled and bedridden patients or end-of-life patients; 3. Patients in acute exacerbation of chronic disease or with sudden other acute conditions; 4. Impaired language expression or communication difficulties, hearing problems, cognitive impairment.
