研究目的：

研究补充硫胺素对慢性心衰患者炎症、氧化应激及心功能的影响。  

Objectives of Study：

The aim of the study was to observe the effects of thiamine supplementation on inflammation, oxidative stress and cardiac function in patients with chronic heart failure.

药物成份或治疗方案详述：

共60例心功能Ⅲ级或Ⅳ级的慢性心力衰竭患者纳入该研究，按1:1的比例分为硫胺素干预组和对照组。硫胺素干预组每日给予肌肉注射硫胺素100mg和呋塞米注射液20mg，并口服盐酸贝那普利片10mg、琥珀酸美托洛尔缓释片23.75mg和螺内酯片20mg；对照组每日给予给予肌肉注射呋塞米注射液20mg，并口服盐酸贝那普利片10mg、琥珀酸美托洛尔缓释片23.75mg和螺内酯片20mg，共干预7天。监测干预前后两组血清总抗氧化能力（T-AOC）、超氧化物歧化酶（SOD）、丙二醛（MDA）、超敏C反应蛋白（hs-CRP）、白介素-6（IL-6）、N末端前脑钠肽（NT-proBNP）、硫胺素含量，左心室射血分数（LVEF）及6分钟步行实验（6MWT）。 

Description for medicine or protocol of treatment in detail：

We will enroll 60 Patients have with NYHA class Ⅲ or Ⅳ chronic heart failure. A 1:1 allocation ratio to receive either thiamine intervention group or control group. Those Patients who are thiamine intervention group were given intramuscular injections thiamine 100mg and fursami 20mg, oral Benazepril Hydrochloride Tablets 10mg. Metoprolol succinate 23.75mg and spironolactone 20mg per day. Those Patients who are control group were given intramuscular injections fursami 20mg,oral Benazepril Hydrochloride Tablets 10mg, Metoprolol succinate 23.75mg and?spironolactone?20mg?per?day. The course is 7 days.Serum T-AOC,SOD,MDA,hs-CRP,IL-6,NT-proBNP,thiamine,and LVEF,6MWT were monitored before and after the study. 

纳入标准：

Inclusion criteria

排除标准：

1.排除患有慢性肾病、糖尿病、甲状腺功能减退、甲亢、急性或慢性感染、慢性肝病、妊娠、癌症和高酒精摄入史的患者。
2.排除合并有其他系统严重原发性或继发性疾病的患者。
3.排除3个月内使用过生物制剂的患者。
4.排除研究者认为不宜进行此项临床试验者。
5.排除依从性差的患者。

Exclusion criteria：

1. Patients who had a history of chronic kidney disease, diabetes, hypothyroidism, hyperthyroidism, acute or chronic infection, chronic liver, pregnancy,cancer,and high alcohol intake were excluded from the the study;
2. Patients combined with severe primary or secondary diseases of other systems;
3. Patients have been using biological agents within 3 months;
4. Patients who are unsuitable for this clinical trial considered by researchers;
5. Patients who with poor compliance were excluded from the study.
