研究目的：

通过单中心随机对照试验，评估经皮耳迷走神经刺激（taVNS）对老年下肢骨科手术患者术后谵妄（POD）发生率的影响，明确taVNS是否能够有效降低POD的发生风险。通过检测患者粪便及血清样本，分析taVNS对肠道菌群及中性粒细胞外诱捕网（NETs）标志物水平的影响，探讨taVNS改善POD的潜在机制是否与调控肠道菌群及NETs形成有关。  

Objectives of Study：

This study aims to evaluate the effect of transcutaneous auricular vagus nerve stimulation (taVNS) on the incidence of postoperative delirium (POD) in elderly patients undergoing lower extremity orthopedic surgery through a single-center randomized controlled trial. The primary objective is to determine whether taVNS can effectively reduce the risk of POD. Secondary objectives include analyzing the effects of taVNS on gut microbiota composition and levels of neutrophil extracellular traps (NETs) markers by detecting patient fecal and serum samples, and investigating whether the potential mechanism of taVNS in improving postoperative delirium is associated with the regulation of gut microbiota and NETs formation.

纳入标准：

Inclusion criteria

排除标准：

1. 拒绝参加本研究； 2.术前存在认知障碍，定义为：基于先前验证的常模及流行病学证据（已考虑教育程度的影响），简易精神状态检查（MMSE）评分低于教育调整后阈值（文盲≤17 分、小学毕业≤20 分、中学毕业≤22 分、高中 / 大学学历≤23 分）； 3.术前耳部有局部皮肤疹、感染、皮肤病变、溃疡或疤痕； 4.存在 taVNS 禁忌证（安装有心脏起搏器、自动植入式心脏除颤器、癫痫或惊厥病史、出血性疾病或抗凝治疗中等）； 5.术前窦性心动过缓（心率＜60 次/min）、严重心脑血管疾病； 6.存在气道高反应性（哮喘、慢性阻塞性通气功能障碍）； 7.孕妇、无法配合治疗的不适合使用taVNS的患者； 8.术前3个月内使用抗生素、益生菌或益生元者； 9.胃肠道手术史，胃肠道疾病（如消化性溃疡、腹泻或胃出血）。

Exclusion criteria：

1.Refuse to participate; 2.Preoperative cognitive impairment, defined as an MMSE score below education-adjustedcutoffs (<=17 for illiterate, <=20 for primary school, <=22 for middleschool, and<=23 for high school/college), based on previouslyvalidated normative and epidemiological evidence accountingfor the influence of education ; 3.Local auricular skin issues (rash, infection, lesions, ulcers, scars) 4.Patients with contraindications for taVNS, including implanted cardiac devices, a history of seizures, bleeding diathesis, or current anticoagulant use 5.Preoperative sinus bradycardia (HR < 60 bpm) and severe cardiovascular or cerebrovascular disease; 6.Airway hyperresponsiveness exists (e.g., asthma, chronic obstructive ventilatory dysfunction) 7.Pregnancy, or unable to cooperate with treatment; 8.Use of antibiotics, probiotics or prebiotics within 3 months before surgery; 9.History of gastrointestinal surgery, gastrointestinal disorders (e.g., peptic ulcer, diarrhea, or gastric bleeding).
