研究目的：

本研究旨在开展一项随机对照临床研究，以评估侧卧自主体位下运用VDLT插、拔管的可行性、安全性及临床优势，主要评价指标包括术后咽痛、插管成功率、DLT移位率、纤支镜辅助率、插管及复定位时间、术中低氧和血压波动发生率，上肢不适等并发症发生率，以及团队主观满意度等，并与传统仰卧插、拔VDLT方法进行比较。我们预期验证“侧卧VDLT插、拔管”在气道稳定性、患者术后恢复及医护团队满意度上的多重优势。  

Objectives of Study：

This study aims to conduct a randomized controlled clinical study to evaluate the feasibility, safety and clinical advantages of using VDLT for intubation and extubation in the lateral lying position. The main evaluation indicators include postoperative pharyngitis, intubation success rate, DLT displacement rate, bronchoscopic assistance rate, intubation and repositioning time, intraoperative hypoxia and blood pressure fluctuation rate, upper limb discomfort and other complications, as well as subjective satisfaction of the team, and compare with the traditional supine VDLT method. We expect to verify the multiple advantages of "lateral VDLT intubation and extubation" in airway stability, patient postoperative recovery and medical team satisfaction.

纳入标准：

Inclusion criteria

排除标准：

1. 有严重心肺疾病； 2. 年龄<18岁或者>75岁； 3. BMI>35kg/m^2； 4. 有认知功能障碍不能配合体位摆动； 5. 有颈椎病史和预期为困难气道（如Mallampati气道分级在4级以上，张口度<3指，甲颏距离<6cm）； 6. 术后送ICU； 7. 术后不能配合随访的病人。

Exclusion criteria：

1. Participants with severe heart or lung diseases; 2. Those under 18 years old or over 75 years old; 3. With a BMI greater than 35 kg/m^2; 4. Those with cognitive dysfunction unable to cooperate with body position changes; 5. Those with a history of cervical spondylosis and an expected difficult airway (such as a Mallampati airway grade of 4 or above, mouth opening less than 3 fingers, and mentohyoid distance less than 6 cm); 6. Those who were sent to the ICU after surgery; 7. Those who could not cooperate with follow-up after surgery.
