本研究旨在系统分析上海市过去10年气候环境因素（如气温、湿度和空气污染）对牙科急性炎症（包括牙周脓肿、根尖脓肿和智齿冠周炎等）的发病影响，并明确相关机制及影响规律。  

Objectives of Study：

This study aims to systematically analyze how climatic environmental factors (e.g., temperature, humidity, and air pollution) have influenced the incidence of acute dental inflammatory conditions (including periodontal abscess, periapical abscess, and pericoronitis of wisdom teeth) in Shanghai over the past decade, and to characterize the related mechanisms and patterns of effect.

Inclusion criteria

排除标准：

1.同一急性发作期间的重复就诊记录（仅纳入每例病例的首诊记录）。
2.缺少关键数据（就诊日期、明确诊断、年龄、性别等信息）者。
3.上海市外籍人士或外地来沪非固定居住者（暴露估计不准确）。

Exclusion criteria：

1.Repeat visits within the same acute episode (only the index/first visit per case will be included).
2.Missing key data (visit date, definitive diagnosis, age, sex, etc.).
3.Foreign nationals in Shanghai or non-resident visitors to Shanghai without a stable local residence (exposure estimates likely inaccurate).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 总体框架：CRF + EDC。 （1）病例记录表（CRF）：依据研究方案统一设计并培训使用，核心字段包括研究编号、就诊日期与类型（门诊/急诊）、性别、年龄、诊断名称与ICD-10编码、诊断科室、病种分型（牙周脓肿/根尖脓肿/智齿冠周炎等），以及用于暴露赋值所需的最小定位信息（如院区/行政区）。 （2）电子数据采集与管理系统（EDC）：院内部署，角色与权限分级，开启审计追踪。内置数据字典与逻辑校验（日期格式、年龄范围、性别取值、诊断—ICD一致性），自动触发**数据疑问（Query）**并记录处理闭环。 2. 数据来源与整合 （1）临床就诊数据：提取上海市口腔医院2016–2025年门/急诊HIS/EHR记录；按诊断名称与ICD-10识别急性口腔炎症病例。 （2）暴露数据：对接同期气象（日均气温、相对湿度）与空气质量（PM₂.₅、NO₂ 等）数据，采用1 km×1 km或同等精度的时空栅格进行匹配。 （3）链接方法：以就诊日为索引日，生成与之匹配的对照日（time-stratified，同年同月同星期几）；依据院区/行政区与栅格中心点进行空间匹配，构建暴露与滞后矩阵供DLNM分析。 3. 数据清洗与去重 依据预设的“安全周期”判定同一急性发作期间的重复就诊，仅保留首诊记录；对缺失/异常值进行范围检查与极端值核对；保留清洗与更改的日志。 4. 质量控制（QC） 双人核查与抽样复核（≥10%）；月度数据质控报告；程序化校验脚本（R/Python）定期运行，包含：缺失率、重复率、分布/离群点、诊断—ICD一致性与时间分层匹配正确性检查。 5. 隐私保护与安全 全流程去标识化，仅保留研究ID；数据静态与传输加密；院内服务器白名单访问与多因素认证；每日增量 + 每周全量备份与异地容灾；按伦理批准的期限保存与销毁。 6. 文档化与版本管理 建立数据字典、CRF版本与分析脚本（DLNM/GAM、条件Logistic）的版本库；记录所有变更与再现性说明；对外仅发布汇总层面结果。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Overall framework: CRF + EDC. (1) Case Record Form (CRF): standardized fields (study ID, visit date/type, sex, age, diagnosis name and ICD-10 code, department, phenotype: periodontal abscess / periapical abscess / pericoronitis, and minimal location for exposure assignment such as campus/district). Staff are trained before data entry. (2) Electronic Data Capture (EDC): hospital-hosted with role-based access control and audit trails. Built-in data dictionary and logic checks (date formats, age ranges, sex codes, diagnosis–ICD consistency). Automatic queries are raised and resolved within the system. 2. Sources and linkage (1) Clinical data: outpatient/emergency HIS/EHR records from 2016–2025; acute oral inflammatory cases identified by diagnosis names and ICD-10 codes. (2) Exposure data: meteorological (daily mean temperature, relative humidity) and air-quality (PM₂.₅, NO₂, etc.) datasets; matched on a 1 km×1 km (or comparable) spatiotemporal grid. (3）Linkage method: the index day is the visit day; control days are selected using a time-stratified scheme (same year/month/day-of-week). Spatial matching uses hospital campus/district to grid centroids; exposure lag matrices are generated for DLNM. 3. Cleaning and de-duplication Apply the protocol-specified “safe period” to identify repeat visits within the same acute episode and retain only the index/first visit. Perform range checks, outlier review, and keep a full change log. 4. Quality control (QC) Dual review with ≥10% sample re-checks; monthly QC reports; routine script-based checks (R/Python) covering missingness, duplicates, distribution/outliers, diagnosis–ICD consistency, and correctness of time-stratified matching. 5. Privacy and security Full de-identification (research ID only); encryption at rest and in transit; allow-listed access with MFA; daily incremental + weekly full backups and off-site disaster recovery; retention and destruction per ethics approval. 6. Documentation and versioning Maintain the data dictionary and CRF versions, plus analysis code repositories (DLNM/GAM, conditional logistic models); record all changes for reproducibility; only aggregated results are released externally.
