研究目的:

随着社会生存压力与日俱增,成年人出现睡眠障碍的比例高达30%。运动员人群其交感神经处于高敏感、高兴奋状态,激素高代谢水平的特点,导致了其出现失眠障碍比例较同年龄段非运动员高1.5倍,且失眠障碍程度更为严重,严重影响运动员训练水平和运动表现。含有巴比妥等成分的安眠药是临床治疗失眠障碍的常用方法,但长期服用此类药物易产生认知功能下降、睡眠结构改变及药物依赖性等问题。此外,对于运动员人群,安眠药部分主要成分是世界反兴奋剂机构(WADA)禁用清单内的禁用物质。因此,寻找改善运动员失眠障碍的非药物性方法具有重要意义。重复经颅磁刺激(repetitive transcranial magnetic stimulation,rTMS)具有改善原发性失眠障碍的作用,但rTMS具有疗程长、干预时间久等局限性,这限制了rTMS在运动员失眠障碍人群中的应用。近年来,在TMS基础上,产生了持续θ脉冲刺激(continuous theta-burst stimulation,cTBS),该方法基于大脑的θ节律(4-8 Hz)进行设计,通过抑制皮层兴奋性实现神经调控。cTBS具有刺激时间短、周期短和延时性好等特点。但cTBS是否可以改善运动员人群的原发性失眠障碍及其神经生理学机制尚不清楚。因此,本研究拟观察cTBS对远动员人群原发性失眠障碍的干预效果,并基于脑电复杂网络模型以全脑网络连接及大脑溯源为切入点,探究cTBS改善运动员失眠障碍的可能机制。  

Objectives of Study:

With the increasing pressure of social survival, the proportion of adults with sleep disorders is as high as 30 %. The sympathetic nerve of athletes is in a state of high sensitivity and high excitement, and the characteristics of high hormone metabolism level lead to the proportion of insomnia disorder 1.5 times higher than that of non-athletes of the same age group, and the degree of insomnia disorder is more serious, which seriously affects the training level and sports performance of athletes. Sleeping drugs containing barbiturates and other components are commonly used in clinical treatment of insomnia disorders. However, long-term use of such drugs is prone to problems such as cognitive decline, sleep structure changes and drug dependence. In addition, for athletes, the main components of sleeping pills are prohibited substances on the World Anti-Doping Agency ( WADA ) banned list. Therefore, it is of great significance to find non-drug methods to improve athletes ' insomnia disorder.Repetitive transcranial magnetic stimulation ( rTMS ) has the effect of improving primary insomnia disorder, but rTMS has the limitations of long course of treatment and long intervention time, which limits the application of rTMS in athletes with insomnia disorder. In recent years, continuous theta-burst stimulation ( cTBS ) has been generated on the basis of TMS.This method is based on the θ rhythm ( 4-8 Hz ) of the brain and achieves neuromodulation by inhibiting cortical excitability. cTBS has the characteristics of short stimulation time, short cycle and good delay. However, whether cTBS can improve primary insomnia disorder in athletes and its neurophysiological mechanism are still unclear. Therefore, this study intends to observe the intervention effect of cTBS on primary insomnia disorder in distant mobilization population, and to explore the possible mechanism of cTBS improving athletes ' insomnia disorder based on the complex network model of EEG with the whole brain network connection and brain traceability as the starting point.

药物成份或治疗方案详述:

刺激装置包括终端设备和8字形TMS线圈。采用cTBS标准模式。簇内频率为50HZ,簇间频率为5HZ。每组600个脉冲。3组cTBS均给予1800次脉冲刺激。每天一次,连续7天,刺激部位为右侧背外侧前额叶。大脑右侧背外侧前额叶的刺激阈值强度无法直接测量,因此采用右侧运动诱发电位( MEP )强度的80 %代替右侧背外侧前额叶的阈值强度进行干预。以拇短展肌为靶肌肉,测试右脑MEP。实验过程中,要求受试者在干预结束后不做剧烈运动,也不主动熬夜,以健康为目的。 

Description for medicine or protocol of treatment in detail:

The stimulation device includes a terminal device and an 8-shaped TMS coil. Using cTBS standard mode. The intra-cluster frequency is 50HZ, and the inter-cluster frequency is 5HZ. 600 pulses per group. Three groups of cTBS were given 1800 pulse stimulation. Once a day for 7 days, the stimulation site was the right dorsolateral prefrontal lobe. The stimulation threshold intensity of the right dorsolateral prefrontal lobe of the brain cannot be directly measured, so 80 % of the right motor evoked potential ( MEP ) intensity is used instead of the right dorsolateral prefrontal lobe threshold intensity for intervention. The right brain MEP was tested with abductor pollicis brevis as the target muscle. During the experiment, the subjects were asked not to do strenuous exercise after the intervention, nor to take the initiative to stay up late, in the interest of health.

排除标准:

由于cTBS干预需要对特定脑区施加磁刺激进行干预,在既往研究中表明,需要排除患有癫痫等精神疾病及颅内有植入金属或人工耳蜗等金属部件等患者,避免在干预过程中对患者造成伤害。同时当匹兹堡睡眠量表得分<7,不满足失眠诊断标准;汉密尔顿焦虑和抑郁量表得分>7时,推测可能是由于消极情绪导致的继发性失眠,故排除。

Exclusion criteria:

Because cTBS intervention requires magnetic stimulation to intervene in specific brain regions, previous studies have shown that patients with mental disorders such as epilepsy and patients with metal components such as implanted metal or cochlear implants need to be excluded to avoid harm to patients during the intervention. At the same time, when the Pittsburgh Sleep Scale score was 7, it was speculated that it may be secondary insomnia caused by negative emotions, so it was excluded.
