建立基于互联网平台（微信）的“患者家庭-社区卫生服务中心-综合性医院”联合照护模式并评价其对超重/肥胖T2DM缓解诱导后维持的效果。  

Objectives of Study：

Establish a joint care model based on the internet platform (WeChat) involving "patient families-community health service centers-comprehensive hospitals" and evaluate its effectiveness in maintaining remission of overweight/obese T2DM after induction.

纳入标准：

Inclusion criteria

排除标准：

1.孕妇、哺乳期女性 2.计划迁移，不能在本研究中心连续随访者 3.合并明确的全身疾病者，包括（但不限于）：曾发生ASCVD（冠心病、缺血性卒中、外周动脉疾病）事件者或心衰、肺栓塞；血液系统疾病：如贫血等；肾功能异常者：血肌酐超过正常值上限，或尿蛋白++（复查后）；肝功能明显异常：转氨酶和（或）总胆红素>=正常上限值的3倍；风湿系统疾病：正应用糖皮质激素和免疫抑制剂治疗者；呼吸系统疾病：严重肺部疾病、目前口服糖皮质激素治疗的患者；内分泌功能异常者：如腺垂体功能不全等；HIV阳性（自我报告）；癌症：过去5年内接受过治疗的癌症患者；器官移植患者；吸毒、酗酒、药物滥用、精神异常等研究者认为不宜参加本研究的其他情况

Exclusion criteria：

1. Pregnant and lactating women 2. Plan to migrate and cannot be followed continuously in the study center 3. Patients with definite systemic diseases, including (but not limited to) history of ASCVD (coronary heart disease, ischemic stroke, peripheral artery disease) events or heart failure or pulmonary embolism; Blood system diseases such as anemia; Abnormal renal function: serum creatinine exceeded the upper limit of normal value, or urine protein ++ (after reexamination); Significant abnormal liver function: transaminase and/or total bilirubin >= 3 times the upper limit of normal; Rheumatic diseases: patients receiving glucocorticoid and immunosuppressive therapy; Respiratory disease: patients with severe lung disease and currently treated with oral corticosteroids; Abnormal endocrine function: such as adenopituitary insufficiency; Hiv-positive (self-reported); Cancer: cancer patients treated within the past 5 years; Organ transplant patients; Drug, alcohol, substance abuse, mental disorders and other conditions that the researcher considered inappropriate to participate in this study

研究干预采用基于互联网平台（微信）的“患者家庭-社区卫生服务中心-综合性医院”联合照护模式管理，管理内容包括：1.确立责任：首诊医生任对符合T2DM缓解基本条件的患者进行评估和实施缓解治疗方案。2.评估：全面了解T2DM 患者的病情，评价患者是否具备缓解条件，有无医疗风险，制定缓解过程中应对医疗风险的预案和防范措施。3.缓解诱导：根据病情的现状，设定缓解目标及与之相匹配的缓解治疗方案。全流程包括患者教育和自我管理能力的培训，并由专业团队通过饮食、营养、运动、药物治疗等措施实现缓解。4.随访：接受缓解治疗的 T2DM患者需要定期进行随访，评估缓解治疗的效果及体质量改善和维持的情况，以及对健康生活方式的依从性。

干预措施代码：

Intervention：

The study intervention was managed by the combined care model of "patients' family, community health service center and general hospital" based on the Internet platform (wechat). The management content included: 1. Establish responsibility: The first visiting physician was responsible for evaluating and implementing remission treatment programs for patients who met the basic conditions for remission of T2DM. 2. Assessment: comprehensively understand the condition of T2DM patients, evaluate whether the patients have the conditions for remission, and whether there are medical risks, and formulate plans and preventive measures to deal with medical risks in the process of remission. 3. Remission induction: according to the status of the disease, set remission goals and corresponding remission treatment regimens. The whole process includes patient education and self-management skills training, and professional teams achieve remission through diet, nutrition, exercise, drug therapy and other measures. 4. Follow-up: Patients with T2DM receiving remission therapy need to be followed up regularly to evaluate the effect of remission therapy, the improvement and maintenance of body weight, and the adherence to healthy lifestyle.
