研究目的：

本研究在循证基础上，结合对患者进行的质性访谈及专家会议，构建以BCW理论为基础的脑卒中后肢体运动障碍患者家庭康复护理方案，使该方案能够有效地指导患者进行家庭康复护理实践。根据BCW理论为基础构建的脑卒中后肢体运动障碍患者家庭康复护理方案，探讨其对患者运动功能、平衡与步态功能、日常生活能力、心理状况以及康复依从性的影响。  

Objectives of Study：

Based on evidence-informed research, qualitative patient interviews, and expert consensus meetings, this study developed a home-based rehabilitation nursing program for patients with limb motor dysfunction after stroke, grounded in the Behavior Change Wheel (BCW) framework. The program aims to effectively guide patients in their home rehabilitation practice. Furthermore, this study investigates the impact of the BCW-based home rehabilitation nursing program on patients' motor function, balance and gait function, activities of daily living, psychological status, and rehabilitation adherence.

Inclusion criteria

排除标准：

1.有精神疾病或认知障碍不能合作的患者； 2.合并患有严重心脑肾等重要脏器疾病的患者； 3.正在接受其他干预研究的患者。

Exclusion criteria：

1. Patients with psychiatric disorders or cognitive impairment that preclude cooperation; 2. Patients with severe comorbidities affecting major organs (e.g., heart, brain, kidneys); 3. Patients currently participating in other interventional studies.
