本研究目的是研究并建立老年人多重用药的管理模式，探索不适宜用药潜在风险的预警方法及干预措施。  

Objectives of Study：

To research and establish elderly patients multi-drug management model, and to explore the waring method and intervention measure of potentially inappropriate medication use risk.

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

(1)有癌症、血液系统疾病等其它严重疾病或预期寿命不足1年；
(2)同时服用呼吸系统、消化系统等非心脑血管系统或非糖尿病药物；
(3)入住ICU的患者。
(4)围术期患者；
(5)有耳聋、智力障碍、严重视力障碍等影响随诊质量的患者；
(6)无法使用app或电话保持稳定联系者；
(7)难以沟通的患者；
(8)长期在外地居住患者

Exclusion criteria：

(1) Patients with cancer, blood system diseases and other serious diseases or life expectancy less than one year;
(2) Taking non-cardio-cerebrovascular or non-diabetic drugs such as respiratory and digestive systems medications at the same time;
(3) Patients who stay in ICU;
(4) Peri-operative patients;
(5) Patients with hearing loss, mental retardation, severe visual impairment and other factors that affect the quality of follow-up.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

源文件定义为纸质CRF表，记录保存5年。由第三方机构建立EDC系统，采用纸质CRF采用双人录入、双人核对的方式录入到EDC系统中。
