研究目的:

1.基于“血管结构-脑区功能-认知表型”框架,构建多维度 ICAS 相关认知障碍层级递进式、数字化三级评估技术谱系; 2.基于队列的随访评估数据和诊断结果应用混合专家技术和搜索推理技术,构建 ICAS 相关认知障碍的深度推理大模型预测模块; 3.基于队列随访评估数据及深度推理大模型,应用蒙特卡洛搜索推理及多模态聚类算法技术,构建 ICAS 相关认知障碍的风险预测和临床分型体系。  

Objectives of Study:

1. Based on the "vascular structure-brain area function-cognitive phenotype" framework, construct a multi-dimensional ICAS-related cognitive impairment hierarchical progression, digital three-level assessment technical spectrum; 2. Use cohort follow-up assessment data and diagnostic results to apply mixed expert techniques and search reasoning techniques, building a deep reasoning large model prediction module for ICAS-related cognitive impairments; 3. Based on cohort follow-up assessment data and the deep reasoning large model, apply Monte Carlo search reasoning and multimodal clustering algorithm techniques to construct a risk prediction and clinical classification system for ICAS-related cognitive impairments.

纳入标准:

Inclusion criteria

排除标准:

1.颅外颈部血管狭窄>50%;
2.有核磁成像的安全性有关的相对/绝对禁忌症(体内金属,幽闭恐惧症等);
3.临床确诊的非粥样硬化病因相关的颅内动脉狭窄,如夹层、血管炎、烟雾病、栓塞、免疫系统疾病等;
4.临床确诊的其他卒中病因亚型,如脑出血、蛛网膜下腔出血、心源性栓塞等;
5.临床确诊的神经系统变性疾病,如阿尔茨海默病、路易体痴呆、额颞叶变性、帕金森病等;
6.其他合并的可能会影响终点事件判断和随访的病史,如脑外伤史、多发性硬化、脑炎、肿瘤、中毒、梅毒以及严重心、肺、肝、肾、内分泌疾病病史;
7.拒绝签署知情同意书;
8.妊娠期或哺乳期妇女不纳入PET/MRI研究。

Exclusion criteria:

1.Extracranial cervical artery stenosis > 50%;
2.Relative/absolute contraindications related to MRI safety (e.g., metallic implants, claustrophobia);
3.Clinically confirmed non-atherosclerotic causes of intracranial stenosis, such as dissection, vasculitis, moyamoya disease, embolism, or immune-mediated diseases;
4.Other confirmed etiological subtypes of stroke, including cerebral hemorrhage, subarachnoid hemorrhage, or cardioembolism;
5.Clinically diagnosed neurodegenerative disorders, such as Alzheimer’s disease, Lewy body dementia, frontotemporal lobar degeneration, or Parkinson’s disease;
6.Other comorbid medical histories that may interfere with endpoint event judgment and follow-up, including history of traumatic brain injury, multiple sclerosis, encephalitis, tumor, poisoning, syphilis, or severe cardiac, pulmonary, hepatic, renal, or endocrine diseases;
7.Refusal to sign informed consent;
8.Pregnant or lactating women will not be included in the PET/MRI substudy.
