评价PRECEDE-PROCEED健康教育护理模式是否能够有效降低非致残性缺血性脑血管病的1年卒中复发风险。  

Objectives of Study：

To evaluate whether the PRECEDE-PROCEED health education and nursing model can effectively reduce the one-year stroke recurrence risk of non-disabling ischemic cerebrovascular disease.

纳入标准：

Inclusion criteria

排除标准：

1. GCS评分＜15分； 2. 已知在住院期间参加了其他干预性临床试验； 3. 因精神疾患、认知或情绪障碍无法理解和/或服从研究程序和/或随访； 4. 有严重的构音障碍或失语，且家属也不能代替问答； 5. 患有严重非心脑血管疾病，预期生存时间小于3个月。

Exclusion criteria：

1. GCS score < 15; 2. Known to have participated in other interventional clinical trials during hospitalization; 3. Unable to understand and/or comply with the research procedures and/or follow-up due to mental disorders, cognitive or emotional impairments; 4. Has severe dysarthria or aphasia, and family members cannot act as questioners and answerers; 5. Suffers from severe non-cardiovascular or cerebrovascular diseases and is expected to survive for less than 3 months.

盲法：

本研究采用单盲设计，以降低测量偏倚：即对随访人员设盲，由独立的随访护士团队负责结局评估（如12个月时的mRS评分、卒中复发、用药情况等），且不告知其患者的具体分组（干预组/对照组）。盲法维护措施：研究数据录入由专门的统计人员完成，不涉及分组信息；在统计分析阶段，数据编码隐藏分组标签，确保结果客观性。

Blinding：

This study adopted a single-blind design to reduce measurement bias: the follow-up personnel were blinded, and an independent follow-up nurse team was responsible for outcome assessment (such as the mRS score at 12 months, stroke recurrence, medication status, etc.), and the specific group (intervention group / control group) of the patients was not informed. Measures for maintaining the blind method: the data entry was completed by specialized statisticians, without involving group information; during the statistical analysis stage, the data encoding concealed the group labels to ensure the objectivity of the results.
