研究目的:

2型糖尿病(Type 2 Diabetes Mellitus, T2DM)是一种慢性进行性的代谢性内分泌疾病,可引发糖尿病心肌病(diabetic cardiomyopathy, DCM),增加心力衰竭风险。强化生活方式干预可缓解或逆转亚临床DCM已是公认事实,但作用机制未明。心脏磁共振(cardiovascular magnetic resonance, CMR)能一站式评价心脏形态、功能、心肌组织特征,实现病理影像化,定量心肌间质纤维化程度。本研究拟利用CMR定量技术对T2DM患者有氧+抗阻运动干预前后DCM的心肌组织特征变化进行评价,为强生活方式缓解DCM的机制提供精准客观证据。  

Objectives of Study:

Type 2 diabetes (T2DM) is a chronic and progressive metabolic endocrine disease, which can cause diabetes cardiomyopathy (DCM) and increase the risk of heart failure. It is widely recognized that intensified lifestyle interventions can alleviate or reverse subclinical DCM, but the mechanism of action is unclear. Cardiac magnetic resonance (CMR) can provide a one-stop evaluation of cardiac morphology, function, and myocardial tissue characteristics, achieving pathological imaging and quantifying the degree of myocardial interstitial fibrosis. This study used CMR quantitative technology to evaluate the changes in myocardial tissue characteristics of DCM in T2DM patients before and after aerobic and resistance exercise intervention, providing accurate and objective evidence for the mechanism of strong lifestyle alleviating DCM.

1.心脏手术史 2.高血压(静息收缩压≥130mmHg或舒张压≥80mmHg) 3.CMR 检查的禁忌症

Exclusion criteria:

1.History of cardiac surgery 2.Hypertension (resting systolic blood pressure ≥ 130mmHg or diastolic blood pressure ≥ 80mmHg) 3.Contraindications for CMR examination

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

通过当面问询或电话采访的方式收集患者临床信息并进行分析。心脏磁共振扫描数据的后处理有两名临床医生分别测量,若果存在差异则请第3位经验丰富的上级临床医师进行评价、校正。数据库建立后,专业的数据分析人员将对数据进行整理、清理、补充和删减。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Collect and analyze clinical information of patients through face-to-face or telephone interviews. The post-processing of cardiac magnetic resonance scan data is measured separately by two clinical doctors. If there are differences, a third experienced superior clinical doctor is requested to evaluate and correct them. After the database is established, professional data analysts will organize, clean, supplement, and delete the data.
