研究目的：

明确在老年髋部手术（髋关节置管术、股骨骨折内固定术）患者中，实施单侧星状神经节阻滞术（SGB）是否能有效维持或提高术中局部脑氧饱和度（rSO2），并减少rSO2显著下降的发生率和/持续时间。探究老年髋部手术中实施SGB是否能有效降低术后谵妄（POD）的发生率。  

Objectives of Study：

This study aims to investigate the efficacy of ultrasound-guided stellate ganglion block (SGB) in maintaining and enhancing regional cerebral oxygen saturation (rSO2) in geriatric patients undergoing hip surgical , while minimizing the incidence and duration of significant rSO2 desaturation. Furthermore, it seeks to explore the potential of SGB implementation in reducing the incidence of postoperative delirium (POD) in geriatric patients.

药物成份或治疗方案详述：

试验组药物成分为：盐酸罗哌卡因，在超声引导下星状神经节阻滞（SGB）操作过程中，将0.5%罗哌卡因7ml注射到C6水平阻滞星状神经节。 

Description for medicine or protocol of treatment in detail：

The pharmacological agent administered to the experimental group was ropivacaine hydrochloride. Under ultrasound guidance, a stellate ganglion block (SGB) procedure was performed, during which 7 mL of 0.5% ropivacaine was injected at the C6 vertebral level into the stellate ganglion (SG).  

纳入标准：

Inclusion criteria

排除标准：

1. 有精神疾病史； 2. 视觉/听觉障碍； 3.术前认知障碍（Mini-Cog<2分）; 4.严重颈动脉狭窄（超声>50%）或SGB禁忌症。

Exclusion criteria：

1. Have a history of mental illness; 2. Visual/auditory impairment; 3. Preoperative cognitive impairment (Mini-Cog<2 points); 4. Severe carotid artery stenosis (ultrasound >50%) or contraindications for SGB.

符合入选标准的患者在签署知情同意书后，将按1:1的比例随机分为SGB组或对照组。训练有素的护士将使用DAP软件（DAP公司，中国北京）在四个参与者的排列块中生成一个分组随机数序列。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Patients who meet the inclusion criteria will be randomly assigned to the SGB group or control group in a 1:1 ratio after signing the informed consent form. A trained nurse will use the DAP software (DAP Co, Beijing, China) to generate a blocked random number sequence in permuted blocks of four participants.

盲法：

Blinding：

Our study is a participant-blinded and assessor-blinded trial. Due to the inconsistent volumes of injected drugs and different block (sham or active), the nurse who dispenses the medication and the anaesthesiologist who performs SGB cannot be blinded. They do not participate in the subsequent study process and are informed that grouping information must not be disclosed. Surgeons, anaesthesiologists performing spinal anaesthesia, nurses conducting postoperative evaluations and statisticians are all blinded to the group allocation.

本研究将采用基于网络的电子数据采集系统进行数据收集，以确保数据的准确性和完整性。 数据采集：麦迪斯顿医疗科技有限公司麻醉系统；大连大学附属新华医院HIS系统。 数据管理：所有数据将直接从源文件录入EDC系统。系统内置逻辑校验功能，并在数据录入过程中进行实时核查。研究结束后，所有结果审核完成，将去标识化后的个体参与者数据将结果公开发表后一年，通过本研究者邮箱获取。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

This research will implement a web-based electronic data capture (EDC) system to ensure data accuracy and integrity. Data Collection: The anesthesia management system provided by Mediston Medical Technology Co., Ltd. and the hospital information system (HIS) of Xinhua Hospital affiliated to Dalian University will be utilized. Data Management: All data will be directly transferred from source documents to the EDC system, which incorporates an integrated logic verification mechanism to perform real-time validation during data entry.
