研究目的：

本研究旨在通过随机对照实验，评估 10°反向Trendelenburg体位联合去甲肾上腺素在耳科手术中术野优化效果，研究成果将为耳科手术出血控制提供更安全的循证方案。主要目的：术野清晰度改善（根据 Modena 出血评分评估手术区域的清晰度），为临床应用优化麻醉方案提供参考。次要目的：术中出血量（通过纱布称重）、手术时间、血流动力学稳定性、术后并发症的发生率，提供临床措施和理论证据。  

Objectives of Study：

This randomized controlled trial aims to evaluate how the 10° reverse Trendelenburg position combined with norepinephrine improves the surgical field during ear surgery.The research results provide a safer, evidence-based solution for bleeding control in ear surgery.The main objective is to improve the clarity of the surgical field, evaluated using the Modena bleeding score, and to provide a reference for optimizing anesthetic management in clinical practice. The secondary objective is to provide clinical measures and theoretical evidence regarding intraoperative blood loss (measured by gauze), operation time, hemodynamic stability, and the incidence of postoperative complications.

纳入标准：

Inclusion criteria

排除标准：

1. 手术时间小于1小时，大于3小时； 2. 术前合并未控制的高血压（SBP>180mmHg）或心律失常； 3. 既往有脑梗病史； 4. 高血压控制不佳及患有严重心脏病； 5. BMI≥30 kg/m²； 6. 慢性疼痛病史或阿片类药物用药史； 7. 其他任何导致不能配合研究的原因，例：语言理解障碍等。

Exclusion criteria：

1.The operation time is either less than 1 hour or more than 3 hours. 2. Preoperative uncontrolled hypertension (SBP>180mmHg) or arrhythmia; 3. History of cerebral infarction; (4) Poorly controlled hypertension and severe heart disease; 4. Poorly controlled hypertension and severe heart disease; 5. BMI >= 30 kg/m²; 6. History of chronic pain or opioid medication; 7. Any other factors preventing cooperation with the research, such as language comprehension disorders.
