研究目的：

评估基于社区开展的由健康教育、运动指导、入户环境评估与改善、药物评估组成的综合干预措施，对以骨质疏松、肌少症为重点疾病的社区老年人跌倒高危人群在干预后1年内预防跌倒发生的效果。次要结局指标包括跌倒知信行水平、跌倒受伤率、跌倒骨折发生率等，为制定社区老年人跌倒预防策略提供科学依据。

Objectives of Study：

This study aims to evaluate the effectiveness of a community-based comprehensive intervention—comprising health education, exercise instruction, home-based environmental assessment and modification, and medication assessments—for preventing falls among high-risk individuals for falls (with a focus on osteoporosis and sarcopenia) among community-dwelling older adults within 1 year post-intervention. Secondary outcome measures include knowledge, attitudes, and practices (KAP) regarding fall prevention, fall injury rate and fall-related fracture incidence. The findings will provide scientific evidence for the development of community-based fall prevention strategies for older adults.

排除标准：

1.处于疾病急性期或慢性疾病急性发作期； 2.听力、视力障碍，或自报患有精神疾病无法正常沟通、交流、学习者； 3.患有不适合运动的其他疾患（医生建议或评估者现场判断）； 4.文盲或者无法完成基本阅读。

Exclusion criteria：

1. Being in the acute phase of an illness or the acute exacerbation phase of a chronic disease; 2. Having hearing or visual impairments, or self-reported mental illness that prevents normal communication, interaction, or learning; 3. Having other medical conditions unsuitable for exercise (as advised by a physician or determined on-site by the evaluator); 4. Illiterate or unable to complete basic reading.
