研究目的：

纤支镜检查是一种刺激性较强的侵入性检查，需要在镜下观察气管和支气管内病变，但气管和支气管内粘液过多会影响观察效果。M3受体主要分布于呼吸道平滑肌、黏膜下腺体、杯状细胞、气道上皮细胞和血管内皮细胞等效应器官，M3受体兴奋分别介导支气管收缩、黏膜下腺体和杯细胞分泌黏液。拮抗M1及M3抗体能够舒张支气管平滑肌并抑制黏液高分泌状态。异丙托溴铵是目前临床上使用最广泛的季铵类抗胆碱能药，可拮抗M3受体。为了提供一个良好的检查操作条件，减少气道分泌物顺利地完成检查以提高纤支镜操作效率和患者舒适度，我们拟采用单盲随机对照试验探索雾化吸入抗胆碱能药物异丙托溴铵是否能减少患者纤支镜过程中呼吸道分泌物，缩短纤支镜操作时间，提高纤支镜操作效率。

Objectives of Study：

Fibrobronchoscopy is a highly irritating invasive examination. It is necessary to observe the trachea and bronchial lesions under the microscope, but too much mucus in the trachea and bronchi will affect the observation. M3 receptors are mainly distributed in the respiratory smooth muscle, submucosal glands, goblet cells, airway epithelial cells and vascular endothelial cells and other effector organs. M3 receptor stimulation mediates bronchoconstriction, submucosal glandular and cup cell secretion of mucus. Antagonist M1 and M3 antibodies can relax bronchial smooth muscle and inhibit mucus hypersecretion. Ipratropium bromide is currently the most widely used quaternary ammonium anticholinergic drugs and can antagonize M3 receptors. In order to provide a good check of operating conditions and reduce airway secretions to successfully complete the examination to improve the efficiency of bronchoscopic surgery and patient comfort, we intend to use a single-blind randomized controlled trial to explore the inhaled anticholinergic drugs ipratropine Whether bromine ammonium can reduce respiratory secretions during bronchoscopy in patients, shorten the operation time of bronchoscopy, and improve the efficiency of bronchoscopic surgery.

Inclusion criteria

排除标准：

＜18岁；行局麻纤支镜患者；不适合用抗胆碱药物的患者如闭角型青光眼或尿潴留患者；对异丙托溴铵，或其他任何组分中的一种，以及阿托品或其衍生物过敏者；妊娠和哺乳期期妇女；在纤支镜检查前已气管插管；之前做过头颈部放疗的；预计全麻行纤支镜的；在本试验前参加过口咽部局部麻醉气雾剂的试验。

Exclusion criteria：

Patients with bronchoscopy; patients not suitable for anticholinergic drugs such as angle-closure glaucoma or patients with urinary retention; ipratropium bromide, or any of the other components, and atropine or Derivative hypersensitivity; Pregnancy and lactation women; Tracheal intubation prior to bronchoscopy; Head and neck radiotherapy performed before; bronchoscopy for general anesthesia; Participation in oropharyngeal localization before this test Anesthetic aerosol test.
