探究深呼吸训练通过激活迷走神经改善脑卒中患者大脑血氧状态的效果,并通过采集治疗前后血流动力学指标(功能近红外光谱技术fNIRS)和心率变异性(HRV)来探究其脑神经机制。  

Objectives of Study:

To explore the effect of deep breathing training on improving cerebral oxygenation in stroke patients by activating the vagus nerve, and to investigate its brain neural mechanisms by collecting hemodynamic indicators (functional near-infrared spectroscopy, fNIRS) and heart rate variability (HRV) before and after treatment.

药物成份或治疗方案详述:

Description for medicine or protocol of treatment in detail:

This study is a crossover study, in which both healthy volunteers and patients underwent the same experimental setup. The setup consisted of two distinct experiments, separated by an interval of at least 48 hours to avoid carryover effects. Experiment 1 involved 20 minutes of deep breathing, while Experiment 2 involved 20 minutes of transcutaneous vagus nerve stimulation (taVNS), and the order of interventions was randomized.Two 5-minute electrocardiogram (ECG) recordings were taken before the intervention to establish the baseline heart rate variability (HRV). After the intervention, three 5-minute ECG recordings were conducted to determine whether there was a direct effect on HRV and whether this effect persisted within 30 minutes after the end of the intervention.A 6-minute resting-state test using functional near-infrared spectroscopy (fNIRS) was performed both before and after the intervention. Data on disease activity and treatment were collected from the patients. All ECG recordings and interventions were conducted in the same sitting position. Active respiratory therapy: The deep breathing exercise consists of a 4-second inhalation followed by a 6-second exhalation, lasting a total of 20 minutes. Initially, participants receive verbal instructions and are guided by visual cues during the practice. Participants are instructed to fully inflate their lungs during inhalation and fully empty them during exhalation. They are free to choose either nasal or oral breathing. Transcutaneous auricular vagus nerve stimulation (taVNS): A transcutaneous auricular vagus nerve stimulator is used. Prior to stimulation, the stimulation site is cleaned with an alcohol swab. In the active taVNS group, electrode-equipped headphones are placed on the left concha of the ear for 20 minutes. Functional near-infrared spectroscopy (fNIRS): A multi-channel continuous-wave NIRS imaging system is used to acquire resting-state and task-state cerebral hemodynamic data. The headcap is positioned such that the lowest row of optodes is aligned along the Fp1–Fp2 line (just above the eyebrows), and the central column of optodes is parallel to the mid-sagittal line (from nasion to inion). Experiments are conducted in a quiet, dimly lit room by personnel trained in fNIRS methodology.

排除标准:

1.健康人排除标准:(1)患有需要定期服用处方药的慢性疾病;(2)心律失常、肺病、无法提供知情同意;(3)有精神疾病史或严重精神疾病(被认为会影响参与研究的能力);(4)成瘾、先前对阿片类药物或兴奋剂成瘾以及怀孕。 2.患者排除标准:(1)存在植入式电子设备、颅内血管夹或其他电子激活/敏感装置;(2)可能干扰刺激或刺激设备的皮肤病变,如瘢痕组织、皮肤破损等;(3)存在严重的心血管系统疾病、或者肺、肝、肾功能不全者;(4)迷走神经术后或迷走神经损伤者;(5)存在除卒中以外其他导致呼吸功能障碍的病因;(6)服用抗抑郁药物或苯二氮卓类药物;(7)过去3个月内注射过肉毒杆菌者;(8)静息心率低于60次/分钟;(9)有癫痫发病史;

Exclusion criteria:

1.Exclusion criteria for healthy individuals:(1) Chronic diseases that require regular prescription medication; (2) Arrhythmia, lung disease, or inability to provide informed consent;(3) History of mental illness or severe mental disorders (that would affect the ability to participate in the study);(4)Addiction, previous opioid or stimulant addiction, and pregnancy. 2.Exclusion criteria for patients:(1) Presence of implanted electronic devices, intracranial vascular clips, or other electronically activated/sensitive devices;(2) Skin lesions that may interfere with stimulation or the stimulation device, such as scar tissue or skin damage;(3) Severe cardiovascular disease, or pulmonary, hepatic, or renal insufficiency;(4)Vagus nerve surgery or vagus nerve damage;(5) Other causes of respiratory dysfunction besides stroke;(6)Use of antidepressants or benzodiazepines;(7) Botulinum toxin injections within the past 3 months;(8)Resting heart rate below 60 beats per minute;(9)History of epilepsy.
