研究目的：

评估奥赛利定联合腹横肌平面阻滞术后镇痛对老年腹部大手术患者术后谵妄发生率的影响，明确该组合治疗在术后5天内对谵妄发生的监测和评估，通过使用3D-CAM和CAM-ICU量表获取数据。记录术后48小时内镇痛泵的有效按压次数、镇痛药物消耗量及相关不良反应，探讨不同镇痛方法对术后患者谵妄发生及疼痛管理效果之间的关系。探讨奥赛利定的作用机制，验证其在减少术后疼痛、降低阿片类药物相关不良反应及神经炎症状态中的重要性，提出基于此联合策略的新型预防措施，以改善老年患者术后预后。  

Objectives of Study：

To evaluate the effect of postoperative analgesia with oxelidine combined with transabdominis plane block on the incidence of postoperative delirium in elderly patients undergoing major abdominal surgery, and to clarify the monitoring and assessment of delirium occurrence within 5 days after surgery by this combination treatment, data were obtained through the use of 3D-CAM and CAM-ICU scales. Record the effective compression times of the analgesic pump, the consumption of analgesic drugs and related adverse reactions within 48 hours after the operation, and explore the relationship between different analgesic methods on the occurrence of delirium and the effect of pain management in postoperative patients. To explore the mechanism of action of oxelidine, verify its importance in reducing postoperative pain, lowering opioid-related adverse reactions and neuroinflammatory states, and propose new preventive measures based on this combined strategy to improve the postoperative prognosis of elderly patients.

纳入标准：

Inclusion criteria

排除标准：

1.既往有精神分裂症、癫痫或帕金森病病史； 2.严重痴呆、语言障碍或患有终末期疾病无法完成术前评估； 3.术前简易智力状态检查量表（MMSE）得分过低（文盲得分 <=17分，小学程度总得分<=20，中学及以上教育程度总得分<=22）； 4.Allen试验阳性，动脉穿刺部位感染、高凝等不能行动脉穿刺者； 5.其他研究者认为不宜纳入研究的情况。

Exclusion criteria：

1. Previous history of schizophrenia, epilepsy, or Parkinson's disease; 2. Severe dementia, language impairment, or terminal disease that cannot complete the preoperative evaluation; 3. The score of the Mini-Mental State Examination Scale (MMSE) before surgery is too low (illiteracy score <=17 points, total score of primary school level <=20, total score of secondary school or above <=22); 4. Those who have a positive Allen test, infection at the arterial puncture site, hypercoagulability, etc., cannot undergo arterial puncture; 5. Other situations that the investigator believes are not suitable for inclusion in the study.

Blind the patients, surgical teams, follow-up personnel, data entry and analysis personnel; Appoint a research coordinator who is responsible for preserving and distributing random numbers, preparing research equipment, and coordinating information among researchers. Researchers responsible for postoperative follow-up and outcome evaluation need to receive training on postoperative delirium assessment. The statistical analysis was completed by the Statistics Office of Liupanshui Vocational and Technical College.
