研究目的：

评估TV-NOTES术式在妇产科领域——子宫全切术、附件切除术、骶骨阴道固定术/骶骨子宫固定术中应用的可行性与安全性；进一步证实TV-NOTES可以作为妇产科领域的一个备选/优选方案，建立相关技术操作指南或治疗规范。  

Objectives of Study：

To evaluate the feasibility and safety of the application of TV-NOTES in the field of Obstetrics and gynecology: total hysterectomy, adnexectomy, sacral vagina fixation/sacral uterus fixation. To further confirm that TV-NOTES can be used as a alternative/preferred option in the field of Obstetrics and Gynecology and to establish a technical guide for the operation and treatment specification.

纳入标准：

Inclusion criteria

排除标准：

1.严重内科合并症难以耐受手术
2.盆底器官脱垂POP-Q I期或无症状
3.阴道狭窄或阴道损伤
4.道格拉斯窝封闭
5.严重子宫内膜异位症
6.有既往盆腹腔手术史（不包括剖宫产）
7.生殖系统、泌尿系统或全身感染的活动期
8.凝血功能障碍
9.未婚/未育
10.确诊或怀疑恶性宫颈、子宫、附件病变
11.妊娠状态
12.附件肿块＞10cm
13.双侧附件切除术

Exclusion criteria：

1. Can not tolerate surgery since the severe internal medicine complication;
2. Stage of POP-Q I or asymptomatic pelvic floor organ prolapse;
3. Vaginal stenosis or vaginal injury;
4. Douglas's nest closure;
5. Severe endometriosis;
6. Had previous history of pelvic and abdominal surgery (excluding cesarean section);
7. Active period of reproductive system, urinary system or systemic infection;
8. Coagulation dysfunction;
9. Unmarried/Unfertile;
10. Diagnose or suspect malignant cervical, uterine and adnexal diseases;
11. State of pregnancy;
12. Adnexal mass > 10cm;
13. Bilateral adnexectomy.
