1.调查北京地区老年心脑血管疾病和糖尿病住院患者多重用药（≥5种以上化学药品）的现状； 2.探索老年人多重用药的不适宜性和潜在风险  

Objectives of Study：

1. To investigate the current status of polypharmacy (using more than 5 types of chemical drugs) in hospitalized elderly patients with cardiovascular and cerebrovascular diseases and diabetes in Beijing; 2. Exploring the inappropriateness and potential risks of multiple medications for the elderly

纳入标准：

Inclusion criteria

排除标准：

①住院时间≤48小时；
②1个月内多次入院；
③从重症监护室转入或转出至重症监护室。

Exclusion criteria：

1. hospitalization time ≤ 48 hours;
2. Multiple admissions within 1 month;
3. Transferred from or to the intensive care unit.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集采用纸质CRF表进行，该文件作为源数据。数据管理采用REDcap EDC 系统。由第三方质控团队进行数据采集和管理的质控。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Paper CRF will be used as a way of raw data collecting. RED cap EDC will be used as tool for data management. An independent quality control group is in charge of whole procedure of data collection and data management.
