研究目的:

本研究旨在评估一种新的经椎弓根撬拨复位(TPR)技术在撑开后的临床和影像学结果,通过Wiltse入路使用顺序分别为单轴-单轴-多轴螺钉配置进行6螺钉固定,用于治疗没有神经功能正在的胸腰椎爆裂骨折。同时关注椎体愈合形态和植入物相关并发症,还将TPR与传统的单纯撑开复位手术方法进行比较  

Objectives of Study:

The aim of this study is to evaluate the clinical and imaging outcomes of a novel transpedicular pry reduction (TPR) technique following distraction in the treatment of thoracolumbar burst fractures without neurological function, using a Wiltse approach with a sequence of monoaxial–monoaxial–polyaxial screw configuration in 6-screw fixation. Particular attention was given to the healing morphology of vertebral bodies and implant related complications, and compare TPR with traditional distraction-only reduction surgical method.

纳入标准:

Inclusion criteria

排除标准:

1. 合并脊髓或神经根损伤(ASIA 分级 A-D 级)。 2. 多节段脊柱骨折或合并其他部位严重创伤需优先处理。 3. 病理性骨折(肿瘤、感染等)。 4. 严重骨质疏松(双能 X 线吸收法骨密度 T 值≤-3.0)。 5. 既往有同节段脊柱手术史。 6. 无法耐受全身麻醉或后路手术。 7. 合并严重心、肺、肝、肾功能不全、凝血功能障碍等手术禁忌症。 8. 精神疾病或认知障碍无法配合研究及随访。 9. 妊娠或哺乳期妇女。 10. 研究者认为不适合纳入的其他情况。

Exclusion criteria:

1. Combined spinal cord or nerve root injury (ASIA grades A-D). 2. Multi segmental spinal fractures or severe injuries in other areas require priority treatment. 3. Pathological fractures (tumors, infections, etc.). 4. Severe osteoporosis (dual energy X-ray absorptiometry bone density T value <= -3.0). 5. Have a history of spinal surgery at the same level in the past. 6. Unable to tolerate general anesthesia or posterior surgery. 7. Surgical contraindications include severe heart, lung, liver, kidney dysfunction, coagulation dysfunction, etc. 8. Unable to cooperate with research and follow-up due to mental illness or cognitive impairment. 9. Pregnant or lactating women. 10. Other situations that researchers consider unsuitable for inclusion.
