研究目的：

（1）探讨初乳灌肠能否改善极早产儿喂养不耐受状况。 （2）探讨初乳灌肠对极早产儿从出生至矫正胎龄40周生长发育的影响。  

Objectives of Study：

(1) To investigate whether colostrum enema can improve feeding intolerance in extremely preterm infants; (2) To examine the effects of colostrum enema on growth and development in extremely preterm infants from birth to corrected gestational age of 40 weeks.

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

（1）合并先天性疾病，如先天性心脏病、先天性消化道畸形等； （2）诊断为严重的消化系统疾病，如肠套叠等； （3) 发生严重感染或其他严重并发症，如脓毒血症、凝血功能障碍等。 （4）产妇存在母乳相关喂养禁忌症，如感染人类免疫缺陷病毒、梅毒、乙型肝炎等；

Exclusion criteria：

(1) Presence of congenital anomalies (e.g., congenital heart disease, congenital gastrointestinal malformations). (2) Diagnosis of severe gastrointestinal disorders (e.g., intussusception, necrotizing enterocolitis). (3) Occurrence of severe infections or life-threatening complications (e.g., sepsis, coagulopathy). (4) Maternal contraindications to breastfeeding (e.g., HIV, syphilis, hepatitis B infection).

由研究负责人使用随机数字表（《医学统计学》附表）从第3行第1列开始横向读取，取随机数末尾数字，按奇偶性分配：奇数—干预组；偶数—对照组。最后，将写有编号、随机数和组别的卡片置于不透明的信封内，由独立研究人员在极早产儿完成基线评估后拆封执行。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The principal investigator allocated participants using a random number table (Appendix Table from Medical Statistics). The allocation sequence was generated starting from Row 3, Column 1, reading horizontally. The last digit of each random number determined group assignment: Odd numbers: Intervention group (colostrum enema) Even numbers: Control group Opaque sealed envelopes containing the assignment information (study ID, random number, and group allocation) were prepared in advance. An independent researcher opened the envelopes only after baseline assessments were completed for each extremely preterm infant.
