研究目的：

本研究旨在依托齐鲁全生命周期电子健康研究型数据库（Cheeloo Lifespan Electronic Health Research Data-library, Cheeloo LEAD）以及27家多中心胆囊结石患者数据，构建大规模胆囊结石手术双向队列，并进一步建立齐鲁胆囊结石手术全生命周期结局数据库（Cheeloo Gallstone Operative Lifespan-outcome Database, Cheeloo GOLD）。研究旨在解决胆囊结石术式选择中的国际性临床争议——即胆囊切除术（LC）远期风险证据不足与胆囊切开取石术（CGPS）术后高复发率的问题，提供创新的解决方案。本研究首次采用模拟目标试验（TTE）技术精准控制混杂偏倚，获取两种术式对胆结石复发、代谢紊乱及恶性肿瘤等结局的高级别循证证据；结合动物实验解析“胆-肠-脑轴”生物学机制，并创新融合因果推断模型与AI大模型技术，构建胆囊手术个性化决策支持智能系统。最终实现“数据-证据-机制-模型”闭环，为国际指南更新提供科学依据，推动胆囊结石的精准治疗。

Objectives of Study：

This study aims to create a large-scale sub-cohort of gallstone surgery patients using data from the Cheeloo LEAD database and the multi-center clinical data of gallstone patients from 27 medical institutions, and to further establish the Cheeloo Gallstone Operative Lifespan-Outcome Database (Cheeloo GOLD) and address the international clinical debate on gallstone surgery, focusing on laparoscopic cholecystectomy (LC) and cholecystolithotomy with gallbladder preservation surgery (CGPS). The study will employ Target Trial Emulation (TTE) techniques to control confounding biases and provide high-grade evidence on long-term outcomes, including stone recurrence, metabolic disorders, and malignancies. Animal experiments will explore the biliary-intestinal-brain axis. Additionally, causal inference and AI modelling will develop a personalized decision support system for gallbladder surgery, ultimately informing clinical guidelines and advancing precision medicine for gallbladder stone disease.

Inclusion criteria

排除标准：

1. 合并胆道相关疾病，包括胆囊结构性疾病、复杂胆道结构性疾病、肝胆管结石、严重胆囊炎、严重胆管炎； 2. 存在胆囊相关手术史：胆囊部分切除史、既往胆囊相关手术史； 3. 重症胰腺炎； 4. 存在恶性肿瘤病史； 5. 存在活动性结核传染病； 6. 合并重要脏器功能不全：严重心功能不全、严重肺功能不全、严重肝功能不全、严重肾功能不全； 7. 合并其他严重疾病：包括严重免疫性疾病、严重凝血功能障碍、严重心功能不全、严重肾功能不全、严重肺功能不全、严重肝功能不全； 8. 存在腹部重大手术史，包肠切除相关手术、胰切除相关手术、胃切除相关手术、脾切除相关手术、膀胱根治性切除/重建术、肝切除相关手术、子宫根治性切除/盆腔廓清术史；

Exclusion criteria：

1. Combined biliary-related diseases, including structural diseases of the gallbladder, complex structural diseases of the bile ducts, hepatobiliary stones, severe cholecystitis, severe cholangitis; 2. History of gallbladder-related surgery: partial cholecystectomy history, previous gallbladder-related surgical history; 3. Severe pancreatitis; 4. History of malignant tumors; 5. Active tuberculosis infection; 6. Combined major organ dysfunction: severe heart dysfunction, severe lung dysfunction, severe liver dysfunction, severe kidney dysfunction; 7. Combined with other severe diseases: including severe immune diseases, severe coagulation disorders, severe heart dysfunction, severe kidney dysfunction, severe lung dysfunction, severe liver dysfunction; 8. History of major abdominal surgery, including bowel resection-related surgery, pancreatic resection-related surgery, stomach resection-related surgery, splenectomy-related surgery, radical bladder resection/reconstruction, liver resection-related surgery, radical hysterectomy/pelvic clearance history;
