研究目的：

1.通过直接支持和间接支持的方式对糖尿病患者进行饮食、运动、自我监测、服药行为等的综合干预，增加患者的认知水平、自我效能，促进患者的自我管理和服药行为，从而达到病情的控制。 2.基于行为阶段变化模型，对直接支持与间接支持对于糖尿病患者行为改变的不同阶段作用机制和作用效果进行探讨，分析并找出行为改变不同阶段最适宜的支持方式。 3.建立一套可推广、可持续、具有长期效果的社区糖尿病直接支持和间接支持干预模式，并在今后尝试推广到其他慢性病的防治和《国家基本公共卫生服务规范》的项目当中。  

Objectives of Study：

1.By means of directive and nondirective support, diabetic patients are intervened in diet, exercise, self-monitoring, medication behavior, etc. to increase their cognitive level and self-efficacy, promote their self-management and medication behavior, so as to achieve disease control. 2.Based on the transtheoretical model, the mechanism and effect of directive and nondirective support on different stages of behavioral change in diabetic patients will be discussed, and the most appropriate support mode for different stages of behavioral change might found out. 3.Establish a set of community diabetes directive and nondirective support intervention model which can be promotional, sustainable and have long-term effect, then try to promote it to other chronic diseases prevention,treatment and the service items of National Basic Public Health Service Standard in the future.

纳入标准：

Inclusion criteria

排除标准：

1.1型糖尿病、妊娠糖尿病、继发糖尿病；
2.合并严重心、脑、肾、眼、足和神经系统并发症(增殖性视网膜病，肾病 IV 期以上或者肌酐>2mg/dl，心功能 III 级以下，有脑血管意外后遗症，糖尿病足 I 级以上)；
3.行动不便、神志不清、精神异常的患者；
4.同时患有肿瘤，近半年内接受放疗、化疗患者；
5.正在参加其他类似研究课题的患者；
6.不愿合作者。

Exclusion criteria：

1. Type I diabetes mellitus, gestational diabetes mellitus and secondary diabetes mellitus;
2. Complicated with serious complications of heart, brain, kidney, eye, foot and nervous system;
3. Patients with inconvenience of movement, mental disorders and mental disorders;
4. At the same time suffering from tumors, patients received radiotherapy and chemotherapy in the past six months;
5. Patients who are participating in other similar research projects;
6. Those who are unwilling to cooperate.
