Objectives of Study：

To explore the effect of establishing evidence-based discharge preparation service for elderly patients with type 2 diabetes on improving discharge readiness and reducing readmission rate.

纳入标准：

Inclusion criteria

排除标准：

1.正在参与其他临床研究；2.合并恶性肿瘤或其他急、危重症；3.有认知功能障碍或患有精神性疾病，无法配合完成干预内容者。

Exclusion criteria：

1. Participating in other clinical studies; 2. Complicated with malignant tumors or other acute, critical and severe conditions; 3. Having cognitive dysfunction or mental illness that prevents them from cooperating with the intervention.

随机方法（请说明由何人用什么方法产生随机序列）：

采用单纯随机分组法，在 Excel 软件中依序输入样本量所需的试验对象编号，另在软件中输入随机公式产生随机数字，根据随机序次等分为 2 组，再将随机数字以升序排列后，即生成序号与组别的对应匹配结果。最后将随机结果制成卡片装入密闭、不透光的信封，封面标注对应序号，根据患者进入研究的先后顺序打开对应信封入组。为了避免两组之间产生沾染，将对照组和观察组进行不同病房的治疗划分。

Randomization Procedure (please state who generates the random number sequence and by what method)：

A simple randomization method was employed. The required sample size of study subjects was sequentially numbered in Excel software. A random number formula was then applied to generate random values, which were used to equally divide participants into two groups based on their random sequence. After sorting the random numbers in ascending order, a matched list of serial numbers and group assignments was generated. The randomization results were then sealed in opaque, consecutively numbered envelopes. As participants were enrolled in the study, their corresponding envelopes were opened in sequential order to assign them to groups. To prevent contamination between groups, the control group and the observation group were treated in separate wards.
